摘要
目的探讨睑板和上睑提肌联合切除治疗重度上睑下垂的有效性及对患者QOL评分的影响。方法选取2016年2月至2018年12月收治的重度上睑下垂患者238例(268眼),依据不同治疗方式分成两组,对照组(119例,134眼)应用额肌悬吊术,研究组(119例,134眼)应用睑板和上睑提肌联合切除术。观察比较两组的近期治疗有效性、术后相关并发症发生情况以及治疗前后的生活质量(QOL)评分变化。结果研究组矫正率为61.9%,显著高于对照组(45.5%,P<0.05)。研究组术后相关并发症发生率为3.0%,显著低于对照组(9.7%,P<0.05)。治疗前,两组QOL各维度评分无显著差异(P>0.05);治疗后,两组各维度评分均显著降低(P<0.05),且研究组较对照组评分更低(P<0.05)。结论应用睑板和上睑提肌联合切除术治疗重度上睑下垂,近期效果显著,术后患者并发症发生率低,可显著提高患者术后的生活质量水平。
Objective To explore the efficacy of combined excision of the levator muscle and tarsus in the treatment of severe ptosis and its effect on QOL score of patients.Methods A total of 238 patients(268 eyes)with severe blepharoptosis from February 2016 to December 2018 were selected,and were divided into two groups according to different treatment method.Patients in control group(119 cases,134 eyes)were treated with frontalis muscle suspension,and patients in study group(119 cases,134 eyes)were treated with combined excision of the levator muscle and tarsus.The recent treatment effectiveness,the occurrence of postoperative complications,and the changes in quality of life(QOL)scores before and after treatment in the two groups were observed and compared.Results The correction rate of the study group was 61.9%,significantly higher than that of the control group(45.5%,P<0.05).The incidence of postoperative complications in the study group was 3.0%,significantly lower than that in the control group(9.7%,P<0.05).Before treatment,there was no significant difference in QOL scores between the two groups(P>0.05).After treatment,the scores of all dimensions in both groups were significantly reduced(P<0.05),and the scores of the study group were lower than those of the control group(P<0.05).Conclusion In the treatment of severe ptosis,the application of combined excision of the levator muscle and tarsus has a significant short-term effect and a low incidence of postoperative complications,which can significantly improve the postoperative quality of life of the patients.
