摘要
妊娠期抗磷脂综合征是高危妊娠,可累及多个器官、系统,对孕产妇及胎儿产生严重影响。近年来该病发病率呈上升趋势,目前尚无标准的治疗方案,虽然阿司匹林、低分子量肝素等药物在临床治疗上已取得一定疗效,新的治疗方法如免疫调节剂、补体抑制剂和靶向炎症反应治疗也处于研究中,但仍需进一步探讨发病机制,深入临床研究来改善这种具有潜在破坏性疾病的治疗策略。中医治疗该病以补肾健脾为基本治法,辨证论治,整体调理,标本兼顾,不良反应小,在临床应用中显示出其独特的优势。但中医药疗法没有统一的指南规范,经验用药和流派占据主导,且缺少大样本、多中心临床随机对照研究,缺乏相关机制的实验研究,需要完善和规范妊娠合并抗磷脂综合征的诊断标准和治疗标准,进行多中心、大样本临床随机对照研究,进行相关机制实验研究,深入探讨中医药疗法的机理。
Antiphospholipid syndrome during pregnancy is a high-risk pregnancy,which can affect multiple organs and systems,and have serious effects on pregnant women and fetuses.In recent years,the incidence of the disease has been on the rise.There is currently no standard treatment plan.Although drugs such as aspirin and low molecular weight heparin have achieved certain effects in clinical treatment,new treatment methods such as immunomodulators,complement inhibitors and targeting inflammatory response treatment is also under research,but it is still necessary to further explore the pathogenesis and in-depth clinical research to improve the treatment strategy of this potentially destructive disease.Chinese medicine treats this disease with the basic treatment method of invigorating the kidney and strengthening the spleen,and the treatment is based on syndrome differentiation and overall regulation,taking into account the manifestation and root cause,which has small adverse reactions,showing its unique advantages in clinical application.However,there is no unified guideline for TCM therapy,and empirical medication and schools dominate.There is a lack of large-sample,multi-center clinical randomized controlled studies,and there is also a lack of experimental studies on related mechanisms.It is necessary to improve and standardize the diagnostic criteria and treatment criteria for pregnancy complicated with antiphospholipid syndrome,conduct multi-center,large-sample clinical randomized controlled studies,conduct experimental research on related mechanisms,and thoroughly explore the mechanism of TCM therapy.
