无创双水平正压通气治疗慢性阻塞性肺疾病急性加重期并Ⅱ型呼吸衰竭患者的临床效果

摘要 目的探讨对慢性阻塞性肺疾病急性加重期(AECOPD)伴有Ⅱ型呼吸衰竭患者行无创双水平正压通气治疗后的临床效果。方法选取云南省曲靖市宣威市第二人民医院2017年2月至2019年3月收治的92例AECOPD伴有Ⅱ型呼吸衰竭患者作为实验对象;信封法分为两组,参照组(46例)给予常规合并症治疗;试验组(46例)给予常规合并症治疗+无创双水平正压通气。比较两组肺功能指标水平[用力肺活量(FEV1/FVC)以及第一秒用力呼气容积(FEV1)]。结果治疗前,两组各肺功能指标水平比较,差异无统计学意义(P>0.05)。治疗后,试验组FEV1/FVC及FEV1均明显高于参照组(P<0.05)。结论AECOPD伴有Ⅱ型呼吸衰竭患者进行无创双水平正压通气治疗后,有利于改善各肺功能指标水平,促进患者早期转归。 Objective To study the effect of noninvasive bi-level positive airway pressure(BiPAP)ventilation for patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and typeⅡrespiratory failure.Methods From February 2017 to March 2019,92 patients with AECOPD and typeⅡrespiratory failure were selected from the Second People's Hospital in Xuanwei City,Qujing City as the subjects,and were assigned to 2 groups by an envelope method.The control group(46 cases)received conventional treatment for complications;while the experimental group(46 cases)received conventional treatment for complications+noninvasive BiPAP ventilation.The levels of lung function[forced vital capacity(FEV1/FVC)and forced expiratory volume in the first second(FEV1)]were compared between the two groups.Results There was no significant difference in lung function levels between the two groups before treatment(P>0.05).After treatment,the FEV1/FVC and FEV1 in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion After noninvasive BiPAP ventilation therapy for the patients with AECOPD and typeⅡrespiratory failure,the lung function was improved,and the early prognosis was promoted.
