摘要
目的探讨早期胃癌诊断治疗中消化内镜黏膜下剥离术的价值,以提高早期胃癌患者的生存质量。方法选取2016年2月~2019年3月于我院就诊的早期胃癌患者60例为研究对象,依据治疗方式不同分为两组,对照组30例予常规诊断治疗,观察组30例予消化内镜黏膜下剥离术,比较两组的临床治疗指标、病理结果、并发症发生率。结果两组患者临床治疗指标比较,观察组各项指标明显优于对照组,差异有统计学意义(P<0.05)。两组患者的病理结果比较,观察组可见1例侧切缘有癌组织残留,经内镜下再次行黏膜下剥离术完整切除病灶;1例水平切缘有癌组织残留,追加腹腔镜手术切除病灶,术后病理未见癌细胞。两组患者有无癌残留及完整切除情况比较,差异无统计学意义(P>0.05)。两组患者并发症发生率比较,观察组显著低于对照组,差异有统计学意义(P<0.05)。结论临床对早期胃癌患者进行诊断治疗时,予以消化内镜黏膜下剥离术能够取得良好的诊断以及治疗效果,同时并发症发生较少,安全有效,值得推广。
Objective To probe into the value of endoscopic submucosal dissection in diagnosis and treatment of early gastric cancer,in order to improve the quality of life of patients with early gastric cancer.Methods 60 cases of patients with early gastric cancer and who treated in our hospital from February 2016 to March 2019 were selected for research and observation,and the patients were divided into two groups according to the different treatment methods.The control group of 30 cases received routine diagnosis and treatment,while the observation group of 30 cases received endoscopic submucosal dissection.The clinical treatment indexes,pathological results and complication rate of two groups were compared.Results The clinical treatment indexes between the two groups were compared,the each indexes in the observation group was significantly better than that in the control group,and the difference between the two groups was significant(P<0.05).The pathological results of two groups were compared,in the observation group,1 case had residual cancer on the lateral margin,and was completely resected by submucosal dissection under endoscopy.1 case had residual cancer on the horizontal margin,and was resected by laparoscopy.No cancer cells were found after operation.There was no significant difference between complete resection and control group(P>0.05).The incidence of complications between the two groups were compared,the observation group was significantly lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion In the clinical diagnosis and treatment of patients with early gastric cancer,the endoscopic submucosal dissection can achieve good diagnosis and treatment results,and the complications are fewer,and it is safe and effective,thus it is worth promoting in the clinical.
