血清HbA1c、TGF-β1及Hcy水平变化与糖尿病肾病的关系研究 被引量:15

摘要 目的探讨血清糖化血红蛋白(Hemoglobin a1c,HbA1c)、转化生长因子β1(Transforming growth factorβ1,TGF-β1)及同型半胱氨酸(Homocysteine,Hcy)与糖尿病肾病(diabetic kidney disease,DKD)的关系。方法选取2017年7月至2019年7月解放军联勤保障部队第九〇四医院76例2型糖尿病患者为T2DM组,93例DKD患者为DKD组,80例健康体检者为健康对照组。检测各组血清HbA1c、TGF-β1及Hcy水平,分析HbA1c、TGF-β1及Hcy表达与DKD发生的关系,探讨3项指标对DKD的诊断价值。结果T2DM组和DKD组患者血清HbA1c、TGF-β1、Hcy水平均显著高于健康对照组,且DKD组血清HbA1c、TGF-β1、Hcy水平又高于T2DM组,差异有统计学意义(P<0.05)。DKD患者血清HbA1c、TGF-β1、Hcy水平与UAER均呈显著正相关关系(P<0.05)。logistic回归分析显示,血清HbA1c、TGF-β1、Hcy升高与DKD的发生呈独立相关性(P<0.05)。结论血清HbA1c、TGF-β1、Hcy高表达是DKD的高危因素,对判断肾脏损伤程度有重要意义。 Objective To investigate the relationship between serum hemoglobin a1c(HbA1c),transforming growth factorβ1(TGF-β1)and homocysteine(Hcy)and diabetic kidney disease(DKD).Methods 76 patients with type 2 diabetes in our hospital from July 2017 to July 2019 were selected as T2DM group,93 patients with DKD were selected as DKD group,and 80 healthy subjects as control group.The levels of HbA1c,TGF-β1 and Hcy were detected in each group.The relationship between the expression of HbA1c,TGF-β1 and Hcy and the occurrence of DKD was analyzed.The diagnostic value of three indexes to DKD was explored.Results Compared with control group,the levels of HbA1c,TGF-β1 and Hcy were significantly increased in T2DM group and DKD group.The levels of serum HbA1c,TGF-β1 and Hcy in DKD group were higher than those in T2DM group(P<0.05).The levels of HbA1c,TGF-β1 and Hcy in DKD patients were significantly positively correlated with UAER(P<0.05).Logistic regression analysis showed that the increase of serum HbA1c,TGF-β1 and Hcy was independently correlated with the occurrence of DKD(P<0.05).Conclusion The high expression of serum HbA1c,TGF-TGF 1 and Hcy are the high risk factors of DKD,which are important to determine the degree of renal injury.
