摘要
目的探讨抗感染联合痰热清注射液治疗肺癌患者放射性肺纤维化(RILF)的疗效及作用机制。方法采用随机数字表法将66例肺癌放疗后伴随RILF的患者分为观察组和对照组,各33例。对照组给予激素、抗感染等常规治疗,观察组在对照组基础上给予痰热清注射液治疗。观察两组患者治疗后的临床疗效,比较两组患者治疗前后的肺功能、抗氧化因子、炎性因子、内皮素1(ET-1)、基质金属蛋白酶9(MMP9)水平。结果观察组患者的治疗有效率高于对照组(P﹤0.05)。治疗后,观察组患者的第1秒用力呼气容积与用力肺活量的比值(FEV1/FVC)、一氧化碳弥散量(DLCO)水平均明显高于对照组(P﹤0.01)。治疗后,观察组患者的血清脂质过氧化物(LPO)水平低于对照组,谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组(P﹤0.05)。治疗后,观察组患者的转化生长因子-β1(TGF-β1)、白细胞介素(IL)-6水平均明显低于对照组,IL-10水平明显高于对照组(P﹤0.01)。治疗后,观察组患者的ET-1水平明显低于对照组,MMP9水平明显高于对照组(P﹤0.01)。结论痰热清注射液联合抗感染治疗肺癌放疗后伴随RILF的患者,有利于纠正氧化与抗氧化的失衡,减轻炎性反应,改善局部微循环,调节MMP9水平,提高临床疗效。
Objective To investigate the efficacy and mechanism of anti-infection combined with Tanreqing injection in the treatment of radiation-induced lung fibrosis(RILF)on patients with lung cancer.Method Sixty-six patients with lung cancer accompanied by RILF after radiotherapy were divided into the observation group and the control group according to the random number table method,with 33 cases in each group.The control group was treated with hormone,anti-infection and other routine treatments,while the observation group was treated with Tanreqing injection on the basis of the control group.After the course of treatment,the clinical efficacy and lung function of the two groups were observed,and the changes of antioxidant factors,inflammatory factors,endothelin-1(ET-1)level and matrix metalloproteinase 9(MMP9)level of patients before and after the treatment in the two groups were detected.Result The effective rate of the observation group was higher than that of the control group(P<0.05).The ratio of forced expiratory volume in one second to forced vital capacity(FEV1/FVC)and diffusion capacity of carbon monoxide of lung(DLCO)in the observation group were significantly higher than those in the control group(P<0.01).After the treatment,the level of lipid peroxidation(LPO)in the observation group was lower than that in the control group,and the level of glutathione peroxidase(GSH-Px)was higher than that in the control group,with statistically significant difference(P<0.05).After the treatment,the levels of transforming growth factorβ1(TGF-β1)and interleukin-6(IL-6)in the observation group were significantly lower than those in the control group,and the level of interleukin-10(IL-10)was higher than that in the control group,with statistically significant difference(P<0.01).After the treatment,the levels of ET-1 in the observation group were significantly lower than those in the control group,while MMP9 levels in the observation group were significantly higher than those in the control group,with statistically significant difference(P<0.01).Conclusion The combination of Tanreqing injection and anti-infection in the treatment of patients with lung cancer accompanied by RILF after radiotherapy is beneficial to improve the oxidation/antioxidant imbalance,reduce the inflammatory response,improve the local microcirculation,regulate the level of MMP9,and improve the clinical efficacy.
