摘要
目的探讨缺血性脑小血管病患者血清NO、ET-1、CysC与血管性认知障碍的相关性。方法选择2018年1月~2019年6月在牡丹江医学院附属第二医院诊断治疗的缺血性脑小血管病患者60例为研究对象,根据蒙特利尔认知评估量表分为无认知障碍组30例与认知障碍组30例。另选择同期健康体检者30例为对照组。比较三组血清NO、ET-1、CysC水平。分析缺血性脑小血管病患者血清NO、ET-1、CysC水平与MoCA评分的相关性。结果(1)无认知功能障碍的患者MoCA高于有认知功能障碍的患者,差异有统计学意义(P<0.05)。(2)认知障碍组血清NO水平显著低于无认知障碍组与对照组,无认知障碍组显著低于对照组,差异均有统计学意义(P<0.05);认知障碍组血清ET-1、CysC水平显著高于无认知障碍组与对照组,无认知障碍组显著高于对照组,差异均有统计学意义(P<0.05)。(3)脑小血管病患者血清NO水平与MoCA评分呈显著正相关,r=0.948。(4)脑小血管病患者血清ET-1、CysC水平与MoCA评分呈显著负相关,r=-0.907、-0.941。结论脑小血管病患者血清NO、ET-1、CysC水平显著发生改变,并且与患者认知功能相关,血清NO与认知功能呈正相关,而血清ET-1、CysC与认知功能呈负相关。
Objective To explore the correlation between serm nitric oxide(NO),endothelin-1(ET-1),cystatin C(CysC)and vascular cognitive impairment(VCI)in patients with ischemic cerebral small vessel disease(ICSVD).Methods 60 patients with ICSVD diagnosed and treated in the Second Affiliated Hospital of Mudanjiang Medical University from January 2018 to June 2019 were selected as the research subjects and divided into the non-cognitive impairment group(n=30)and the cognitive impairment group(n=30)according to Montreal Cognitive Assessment(MoCA)scale.Another 30 cases of health proved by check-up in the same period were selected as the control group.The levels of serum NO,ET-1 and CysC in the three groups were compared.The correlation between serum NO,ET-1,CysC levels and MoCA scores in the patients with ICSVD was analyzed.Results(1)The MoCA scores of the patients without cognitive impairment were higher than those of the patients with cognitive impairment,and the difference was statistically significant(P<0.05).(2)The level of serum NO in the cognitive impairment group was significantly lower than that in the non-cognitive impairment group and the control group,while that in the non-cognitive impairment group was significantly lower than that in the control group,and the differences were statistically significant(P<0.05);The levels of serum ET-1 and CysC in the cognitive impairment group were significantly higher than those in the non-cognitive impairment group and the control group,while those in the non-cognitive impairment group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).(3)There was a significant positive correlation between serum NO level and MoCA scores in the patients with cerebral small vessel disease(CSVD),r=0.948.(4)The levels of serum ET-1 and CysC in the patients with CSVD were significantly negatively correlated with the MoCA scores,r=-0.907,-0.941.Conclusion The levels of serum NO,ET-1 and CysC in the patients with CSVD have significantly changed and are correlated with the cognitive function of the patients.Serum NO is positively correlated with cognitive function,while serum ET-1 and CysC are negatively correlated with cognitive function.
