摘要
目的通过分析眼眶黏膜相关淋巴组织边缘区B细胞淋巴瘤(MALT淋巴瘤)3.0T1H-MRS波谱图像特点,以探讨3.0T1H-MRS对眼眶MALT淋巴瘤的诊断价值。方法选取我院2009年1月~2019年1月收治的眼眶MALT淋巴瘤患者33例(50眼)进行研究(患者均得到病理及免疫组化验证),对眼眶MALT淋巴瘤感兴趣区均实施3.0T MRI、3.0T1H-MRS检测,并和同侧额部正常脑组织3.0T1H-MRS所测生化指标进行对比分析,主要分析乙酸(Ace)、乳酸(Lac)、N-乙酰天门冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr)峰值及波峰积分面积(积分数值),得到各生化指标相对值:Cr/Lac、Lac/NAA、Cho/NAA、Cr/NAA、Cho/Cr、和Lac/Ace。结果Lac峰积分数值上升最为明显(33.20±12.00)与正常脑组织比较,差异有统计学意义(t=20.290,P=0.000);Cho峰积分数值下降比较明显(2.85±0.05)与正常脑组织比较,行t检验,有统计学差异(t=20.146,P=0.000);NAA、Cr峰积分数值下降也比较明显,也具有统计学意义。Cr/NAA积分数值比值上升最为明显(1.26±0.18)与正常脑组织比较,行t检验,差异有统计学意义(t=17.439,P=0.000);Cho/NAA积分数值比值上升最为明显(2.11±1.09)与正常脑组织比较,行t检验,差异有统计学意义(t=9.664,P=0.000);Lac/NAA积分数值比值上升较为明显(24.59±20.82),与正常脑组织比较,行t检验,差异有统计学意义(t=8.265,P=0.000);Lac/Ace、Cho/Cr积分数值比值虽然上升,但无统计学意义。结论3.0T1H-MRS对眼眶MALT淋巴瘤的波谱分析具有明显生化特性:Lac峰积分数值明显升高,Cr/NAA积分数值比值上升最为明显,Cr/Lac峰积分数值比值明显下降,Lac/NAA比值明显上升。这些可作为诊断眼眶MALT淋巴瘤的重要参考依据。
Objective To analyze the characteristics of 3.0T1H-MRS spectral image of marginal zone B-cell lymphoma of orbital mucosa-associated lymphoid tissue(MALT),and to explore the diagnostic value of 3.0T1H-MRS for orbital MALT lymphoma.Methods Thirty-three patients(50 eyes)with orbital MALT lymphoma admitted to our hospital from January 2009 to January 2019 were selected for study(all patients were confirmed by pathology and immunohistochemistry),and all orbital-interested areas of MALT lymphoma were detected by 3.0T MRI,3.0T1H-MRS,and were compared with the biochemical indicators of normal brain tissue in the ipsilateral frontal region measured by 3.0T1H-MRS.The acetic acid(Ace),lactic acid(Lac),N-acetylaspartic acid(NAA),choline(Cho),creatine(Cr)peak and peak integration area(integral value)were analyzed to get the relative values of various biochemical indicators:Cr/Lac,Lac/NAA,Cho/NAA,Cr/NAA,Cho/Cr,and Lac/Ace.Results The Lac peak integral value increased most significantly(33.20±12.00)as compared with normal brain tissue,the difference was statistically significant(t=20.290,P=0.000);the Cho peak integral value decreased significantly(2.85±0.05)as compared with normal brain tissue,and the t test showed that the difference was statistically significant(t=20.146,P=0.000);the NAA and Cr peak integral values also decreased significantly,and the differences were also statistically significant.The ratio of Cr/NAA integral value increased most obviously(1.26±0.18)as compared with normal brain tissue,and the t test showed that the difference was statistically significant(t=17.439,P=0.000);the ratio of Cho/NAA integral value increased most obviously(2.11±1.09)as compared with normal brain tissue,and the t test showed that the difference was statistically significant(t=9.664,P=0.000);the ratio of Lac/NAA integral value increased significantly(24.59±20.82)as compared with normal brain,and the t test showed that the difference was statistically significant(t=8.265,P=0.000);although the ratios of Lac/Ace,Cho/Cr integral values increased,the differences were not statistically significant.Conclusion 3.0T1H-MRS has obvious biochemical characteristics for spectroscopic analysis of orbital MALT lymphoma:Lac peak integral value increases significantly,Cr/NAA integral value ratio increases most obviously,Cr/Lac peak integral value ratio decreases significantly,and Lac/NAA ratio significantly increases.These can be served as important references for the diagnosis of orbital MALT lymphoma.
