摘要
目的探讨基于穴位按摩的早期综合干预对早产脑损伤患儿神经发育的影响。方法回顾性分析2016年3月至2018年3月宝鸡市妇幼保健院收治的120例早产脑损伤患儿的临床资料,根据干预方法不同分为观察组(n=60)和对照组(n=60)。对照组采用传统早期综合干预;观察组采用基于穴位按摩的早期综合干预。两组均干预至12月龄。分析两组干预前后神经行为评分及发育商评分变化情况;两组治疗后均随访6个月,分析两组随访期间后遗症发生情况。结果一般评估、主动肌张力、被动肌张力、原始反射及行为能力评分的组间和时点间存在交互作用(P<0.01),两组干预前后各指标的变化幅度不同,干预6月龄、干预12月龄高于干预前,干预12月龄高于干预6月龄,且干预6月龄、干预12月龄观察组高于对照组(P<0.05)。大运动、精细动作、适应能力、语言、社交能力评分的组间和时点间存在交互作用(P<0.05),两组干预前后各指标的变化幅度不同,干预6月龄、干预12月龄高于干预前,干预12月龄高于干预6月龄,且干预6月龄、干预12月龄观察组高于对照组(P<0.05)。观察组患儿总后遗症发生率低于对照组[5.17%(3/58)比17.54%(10/57)](P<0.05)。结论基于穴位按摩的早期综合干预可改善早产脑损伤患儿的神经行为和智能发育,降低后遗症发生风险。
Objective To explore the effect of early comprehensive intervention based on acupoint massage on neural development of preterm infants with brain injury.Methods The clinical data of 120 cases of premature brain injury in Baoji Maternal and Child Health Hospital from Mar.2016 to Mar.2018 were collected for retrospective analysis.According to different intervention methods,the 120 patients were divided into an observation group(n=60)and a control group(n=60).The control group was treated with traditional early comprehensive intervention;the observation group was treated with early comprehensive intervention based on acupoint massage.Both groups were intervened for 12 months.The changes of neonatal behavioral neurological assessment and developmental quotient score before and after the intervention in the two groups were analyzed.The patients in both groups were followed up for 6 months after treatment.The incidence of sequelae during the follow-up was analyzed.Results There were interactions in general assessment,active and passive muscle tone,and primitive reflex and behavioral ability score between group and time point(P<0.01),the index changing amplitudes of the two groups before and after the intervention were different,6 months intervention,12 months intervention were higher than before the intervention,12 months intervention was higher than 6 months intervention,6 months intervention,12 months intervention of the observation group were higher than the control group(P<0.05).There were interactions in macro movement,fine movement,adaptive ability,language,social ability score between group and point(P<0.05),the index changing amplitudes of the two groups before and after intervention were different,6 months intervention,12 months intervention were higher than before the intervention,12 months intervention was higher than 6 months intervention,and 6 months intervention and 12 months intervention of the observation group were higher than the control group(P<0.05).The total incidence of sequelae in the observation group was lower than that in the control group[5.17%(3/58)vs 17.54%(10/57)](P<0.05).Conclusion Early comprehensive intervention based on acupoint massage can improve the neurobehavioral and intellectual development of preterm children with brain injury,and reduce the risk of sequelae.
