摘要
目的探讨补肾活血汤对骨质疏松性椎体压缩性骨折经皮椎体后凸成形术后残余疼痛的改善效果。方法将我院骨外科2017年1月至2019年6月收治的115例骨质疏松性椎体压缩性骨折患者按照随机数字表法分为对照组(55例)和观察组(60例)。两组均实施经皮椎体后凸成形术,手术后,对照组给予常规阿法骨化醇、碳酸钙D3治疗;观察组在对照组基础上给予补肾活血汤。比较两组治疗效果。结果治疗后,两组前臂、腰椎正位、股骨颈的骨密度和N-MID、TPINP水平均显著升高,且观察组高于对照组(P<0.05)。治疗后,两组伤椎高度比值均增大,Cobb角均减小,且观察组优于对照组(P<0.05)。治疗后,两组疼痛、脊柱功能障碍评分均降低,且观察组低于对照组(P<0.05)。结论骨质疏松性椎体压缩性骨折患者经皮椎体后凸成形术后给予补肾活血汤治疗,可更好地增加骨密度,改善骨代谢情况,有利于维持术后椎体稳定,减轻术后疼痛感和脊柱功能障碍。
Objective To investigate the improvement effect of Bushen Huoxue decoction on residual pain after percutaneous kyphoplasty for osteoporotic vertebral compression fracture.Methods A total of 115 patients with osteoporotic vertebral compression fracture admitted in our hospital from January 2017 to June 2019 were divided into control group(55 cases)and observation group(60 cases)according to random number table method.The two groups were treated with percutaneous kyphoplasty,after operation,the control group was treated with conventional alfacalcidol and calcium carbonate D3;and the observation group was treated with Bushen Huoxue decoction on the basis of the control group.The treatment effects were compared between the two groups.Results After treatment,the bone mineral density of forearm,lumbar spine and femoral neck and the levels of N-MID,TPINP in the two groups significantly increased,and those of the observation group were higher than the control group(P<0.05).After treatment,the ratio of injured vertebrae height increased and Cobb angle decreased in both groups,and those of the observation group were better than the control group(P<0.05).After treatment,the pain and spinal dysfunction scores in the two groups decreased,and those of the observation group were lower than the control group(P<0.05).Conclusion Bushen Huoxue decoction can increase bone mineral density and improve bone metabolism in patients with osteoporotic vertebral compression fracture after percutaneous kyphoplasty,which is conducive to maintain the stability of vertebral body,reduce postoperative pain and spinal dysfunction.
