维持性血液透析患者血清甲状旁腺激素、尿素氮、尿酸水平与预后的关系 被引量:4

The relationship between serum PTH,urea nitrogen and uric acid levels and the prognosis of patients undergoing maintenance hemodialysis

摘要 目的探讨维持性血液透析(MHD)患者血清甲状旁腺激素(PTH)、尿素氮(BUN)、尿酸(SUA)水平与预后的关系。方法我院肾内科接受MHD患者103例,收集入院时人口统计学与生化指标测定结果。患者根据预后结果分为预后良好组和预后不良组,并进行统计学分析。结果随访时长(23.55±5.71)月,预后不良者36例(34.95%)。与预后良好组相比,预后不良组年龄、透析龄及血清PTH、BUN、SUA水平更高(P<0.05),性别、体质量指数(BMI)、原发病类型及总胆固醇、甘油三酯、血磷、血钙、血红蛋白等比较,差异无统计学意义(P>0.05)。透析龄及血清PTH、BUN、SUA水平是影响MHD患者预后的独立危险因素(P<0.05)。ROC曲线分析显示血清PTH、BUN、SUA均可预测MHD患者不良预后,其曲线下面积分别为0.900、0.787、0.843。结论血清PTH、BUN、SUA水平升高是MHD患者不良预后的危险因素,且对预测预后有一定价值。 Objective To investigate the relationship between serum parathyroid hormone(PTH),urea nitrogen(BUN)and uric acid(SUA)levels and the prognosis of patients undergoing maintenance hemodialysis(MHD).Methods The clinical data of 103 patients undergoing MHD at the nephrology department of our hospital were collected.The measurement results of demographic and biochemical indicators were statistically analyzed.All patients were followed up and they were divided into good prognosis group and poor prognosis group.Statistical analysis was performed.Results The subjects were followed up with an average of 23.55±5.71 months.There were 36 patients(34.95%)with poor prognosis.Compared with the good prognosis group,the age,dialysis age,serum PTH,BUN and SUA levels in the poor prognosis group were significantly older,longer or higher(P<0.05).However,there was no significant difference in gender,body mass index(BMI),primary disease type,total cholesterol,triglyceride,serum phosphorus,blood calcium or hemoglobin(P>0.05).Dialysis age and levels of serum PTH,BUN and SUA were independent risk factors affecting the prognosis of patients undergoing MHD(P<0.05).ROC curve analysis showed that serum PTH,BUN and SUA could be used to predict the poor prognosis of patients undergoing MHD.The areas under the curve were 0.900,0.787 and 0.843,respectively.Conclusion Increased serum PTH,BUN and SUA levels are risk factors for poor prognosis of patients undergoing MHD.They have a certain value in predicting prognosis.
