糖尿病酮症酸中毒伴高血糖高渗状态的抢救与治疗效果分析 被引量:8

摘要 目的探究在糖尿病酮症酸中毒伴高血糖高渗状态患者抢救与治疗效果。方法选取2017-01至2019-12在我院收治的糖尿病酮症酸中毒伴高血糖高渗状态患者62例为研究对象,以电脑随机数字表法分为对照组和观察组,各31例。对照组采用常规抢救治疗,观察组采用优化抢救治疗。比较两组患者救治效果、低血糖发生率、治疗前后心肌酶谱水平及炎症因子水平。结果观察组患者血糖达标时间、纠正高渗时间均较对照组短;观察组低血糖发生率为3.23%较对照组25.81%低;治疗后观察组谷草转氨酶(aspartate aminotransferase,AST)、乳酸脱氢酶(lactate dehydrogenase,LDH)、肌酸激酶(creatine kinase,CK)、肌酸激酶同工酶(Creatine Kinase MB,CK-MB)水平较对照组低;治疗后观察组C反应蛋白(C-reactive protein,CRP)、白细胞介素-6(Interleukin-6,IL-6)、肿瘤细胞坏死因子-α(TNF-α)水平较对照组低,差异均具有统计学意义(P<0.05)。结论对糖尿病酮症酸中毒伴高血糖高渗状态患者应用优化抢救治疗方案,可缩短病程、降低低血糖发生率、改善患者心肌功能、缓解炎症反应,效果理想。 Objective To explore the rescue and treatment effects of patients with diabetic ketoacidosis with hyperglycemia and hyperosmolarity.Methods A total of 62 patients with diabetic ketoacidosis and hyperglycemia and hyperosmolarity who were admitted to our hospital from January 2017 to December 2019 were selected as the research objects.They were divided into a control group and a observation group by a computerized random number table,with 31 cases in each.The control group received conventional rescue treatment,and the observation group adopted optimized rescue treatment.The treatment effect,incidence of hypoglycemia,myocardial enzyme spectrum level and inflammatory factor levels before and after treatment were compared between the two groups.Results The time to reach the standard of blood glucose and the time to correct hypertonicity in the observation group were shorter than those in the control group;the incidence of hypoglycemia in the observation group was 3.23%,which was lower than 25.81%in the control group;after treatment,the observation group aspartate aminotransferase(AST)and lactate dehydrogenase(LDH),creatine kinase(CK),and creatine kinase MB(CK-MB)levels were lower than those of the control group;C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)were lower than those of the control for group,and the differences were statistically significant(P<0.05).Conclusions The application of optimized rescue treatment plan to patients with diabetic ketoacidosis and hyperglycemia and hyperosmolar state can shorten the course of the disease,reduce the incidence of hypoglycemia,improve the patient’s myocardial function,and alleviate inflammation,with ideal results.
