子宫内膜癌组织中miR-21、STAT3、FOXO3a的表达及临床意义 被引量:9

Expression and clinical significance of miR-21, STAT3 and FOXO3a in endometrial cancer

摘要 目的观察子宫内膜癌组织中微小RNA(miR-21)、信号转导和转录激活因子3(STAT3)、叉头框蛋白O3a(FOXO3a)表达特点,分析其与子宫内膜癌临床病理参数、预后的关系。方法选取联勤保障部队第九八八医院2014年2月至2016年10月妇产科行经腹广泛或次广泛性全子宫切除术+盆腔淋巴结清扫术切除,且经病理首次确诊为子宫内膜癌组织标本92例为子宫内膜癌组,另外选取门诊体检经宫腔镜取组织活检病理证实的正常子宫粘膜组织标本60例为对照组。应用实时RT-PCR技术分别检测子宫内膜癌及正常子宫膜中miR-21、STAT3、FOXO3a表达,并随访至2019年1月,统计分析上述三者与子宫内膜癌临床病理参数、预后的关系。结果子宫内膜癌组miR-21、STAT3相对表达量高于对照组,FOXO3a则显著降低,差异均有统计学意义(P<0.001)。子宫内膜癌组miR-21、STAT3表达水平随着TNM分期增加、组织分化程度降低、浸润深度增加、淋巴转移而增高,FOXO3a下降,差异均有统计学意义(P<0.05),三者子宫内膜癌临床病理分型差异有统计学意义(P<0.05)。miR-21表达水平与STAT3表达水平呈正相关,FOXO3a与miR-21、STAT3表达水平呈负相关,差异均有统计学意义(P<0.05)。高表达miR-21、STAT3子宫内膜癌患者生存率低于低表达miR-21、STAT3组,差异有统计学意义(P<0.05),低表达FOXO3a组患者生存时间低于高表达FOXO3a组,差异有统计学意义(P<0.05)。结论 miR-21、STAT3子宫内膜癌中高度表达,FOXO3a表达下调,miR-21、STAT3、FOXO3a表达水平与肿瘤侵袭转移等生物学行为密切相关,可作为临床诊断和预后预测的生物学指标。 Objective To observe the expression characteristics of microRNA-21(miR-21),signal transduction and transcription activator 3(STAT3),forkhead box protein O3a(FOXO3a) in endometrial cancer tissues,and analyze their relationship with clinicopathological parameters and prognosis of endometrial cancer.Methods From February 2014 to October 2016,92 patients with endometrial carcinoma were selected from the Department of Obstetrics and Gynaecology in 988 Hospital of the Joint Logistic Support Force who underwent extensive or sub-wide hysterectomy for pelvic lymphadenectomy and were first diagnosed by pathology as endometrial carcinoma group.In addition,60 samples of normal uterine mucosa confirmed by hysteroscopy were selected as control group.Real time RT-PCR was used to detect the expression of miR-21,STAT3 and FOXO3a in endometrial carcinoma and normal endometrium.The relationships between miR-21,STAT3 and FOXO3a expressions and the clinicopathological parameters and prognosis of endometrial carcinoma were statistically analyzed.Results The relative expressions of miR-21 and STAT3 in endometrial carcinoma group was higher than those in control group,while FOXO3a was significantly lower(P<0.001).The expression levels of miR-21 and STAT3 in endometrial carcinoma group increased with the increase of TNM stage,the decrease of tissue differentiation,the increase of infiltration depth and lymph node metastasis,and the FOXO3a decrease(P<0.05).There were significant differences between the three groups and clinicopathological classification(P<0.05).The expression level of miR-21 was positively correlated with that of STAT3,while FOXO3a was negatively correlated with that of miR-21 and STAT3.The survival rate of patients with high expression levels of miR-21 and STAT3 was lower than that of patients with low expression levels of miR-21 and STAT3(P<0.05),and the survival time of patients with low expression level of FOXO3a was lower than that of patients with high expression level of FOXO3a(P<0.05).Conclusion The expression levels of miR-21 and STAT3 were high in endometrial cancer,and FOXO3a was down-regulated.The expression levels of miR-21,STAT3 and FOXO3a were closely related to the biological behavior of tumor invasion and metastasis,which could be used as biological indicators for clinical diagnosis and prognosis prediction.
