OCT应用于伴有IR的兔颈总动脉斑块特征的诊断价值研究

摘要 目的探讨光学相干断层成像(optical coherence tomography,OCT)评价伴有胰岛素抵抗(insulin resistance,IR)的兔颈动脉斑块特征的价值。方法选取普通级雄性新西兰大白兔26只,随机抽取6只白兔作为对照组,剩余20只白兔作为观察组,对照组给予高脂喂养,观察组给予颈总动脉内膜损伤术以及高脂喂养,采用OCT观察各组颈总动脉中膜厚度、内膜厚度和斑块厚度,同时与病理结果进行比较;检测各组胰岛素抵抗指数(insulin resistance index,HOMA-IR)。结果病理结果显示:观察组斑块厚度为(520.00±112.20)μm,明显高于对照组(P<0.05),观察组和对照组颈动脉内膜、中膜厚度比较,差异无统计学意义(P>0.05);OCT检测与病理显示斑块个数及性质吻合;观察组、对照组OCT与病理测量颈动脉内膜厚度、中膜厚度和斑块厚度比较,差异无统计学意义(P>0.05);观察组HOMA-IR为(70.33±2.18),明显高于对照组(P<0.05);观察组HOMA-IR与斑块厚度呈正相关(r=0.751,P<0.05)。结论 OCT在评价兔颈总动脉斑块性质及厚度有较高的可靠性,同时HOMA-IR与斑块厚度有关。 Objective To investigate the value of optical coherence tomography( OCT) in evaluating the characteristics of carotid plaque in rabbits with insulin resistance( IR). Methods 26 ordinary male New Zealand rabbits were selected,6 white rabbits were randomly selected as control group( CG),and the remaining 20 white rabbits were used as observation group( OG). CG was fed with high-fat diet,while OG was given with carotid endarterectomy and high-fat diet. The thickness of carotid artery media,intima and plaque were observed by OCT,pathological results were compared,and insulin resistance index( HOMA-IR) was measured. Results Pathological results showed that the plaque thickness in OG was( 520. 00 ±112. 20) μm,which was significantly higher than that in CG( P< 0. 05). There were no significant differences in carotid intima-media thickness between OG and CG( P< 0. 05). OCT and pathological examination showed that the number and nature of plaques were consistent. There were no significant differences in carotid intima thickness,intima media thickness and plaque thickness of OCT and pathological measurement between OG and CG( P>0. 05).The HOMA-IR of OG was( 70. 38±2. 18),which was significantly higher than that of CG( P<0. 05).TheHOMA-IR of OG was positively correlated with plaque thickness( r = 0. 751,P<0. 05).Conclusion OCT has high reliability in evaluating the plaque properties and thickness in rabbitscommon carotid artery,and HOMA-IR is related to plaque thickness.
