摘要
目的探讨肌电生物反馈联合电针、穴位注射交替治疗外伤性尺神经损伤的临床效果。方法回顾性分析2016年1月~2019年8月我科收治52例外伤性尺神经损伤患者临床资料,按不同治疗方式分为观察组和对照组,每组各26例。对照采用甲钴胺片口服、神经肌肉电刺激、运动疗法、电针与穴位注射交替进行治疗,观察组在此基础上加予肌电生物反馈治疗,经过3个疗程(60 d)治疗,比较两组患者治疗后手的外形、关节总主动活动度(TAM)、感觉恢复情况,同时检测患侧尺神经的神经传导速度、波幅等电生理指标。结果治疗后,观察组患者的优良率(84.62%)高于对照组(57.69%),差异有统计学意义(P<0.05)。治疗前,两组患者的神经传导速度和波幅比较,差异无统计学意义(P>0.05);治疗后两组患者的神经传导速度和波幅均高于治疗前,且观察组治疗后的神经传导速度和波幅均高于对照组,差异有统计学意义(P<0.05)。结论肌电生物反馈联合电针与穴位注射交替治疗外伤性尺神经损伤具有良好的协同作用,可促使外伤性尺神经损伤后形态和功能恢复。
Objective To explore the clinical effect of electromyography biofeedback combined with electroacupuncture and acupoint injection in the treatment of traumatic ulnar nerve injury.Methods A retrospective analysis of 52 patients with traumatic ulnar nerve injury admitted to our department from January 2016 to August 2019 was made.They were divided into observation group and control group according to different treatment methods,with 26 cases in each group.The control group was treated with Mecobalamin Tablets orally,neuromuscular electrical stimulation,exercise therapy,electroacupuncture and acupoint injection alternately,the observation group was treated with electromyography biofeedback on this basis.After 3 course(60 days)of treatment,the shape of the hands,total active movement(TAM),and sensory recovery of patients were compared in the two groups and electrophysiological indicators such as nerve conduction velocity and wave amplitude of the affected ulnar nerve were measured.Results After treatment,the excellent and good rate in the observation group(84.62%)was higher than that in the control group(57.69%),the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in nerve conduction velocity and wave amplitude of patients between the two groups(P>0.05);after treatment,the nerve conduction velocity and wave amplitude of patients of the two groups were higher than those before treatment,and the nerve conduction velocity and wave amplitude of the observation group after treatment were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion Electromyography biofeedback combined with electroacupuncture and acupoint injection in the treatment of traumatic ulnar nerve injury has a good synergistic effect,which can promote the recovery of morphology and function after traumatic ulnar nerve injury.
