羟丙基甲基纤维素预处理辅助超声乳化人工晶状体植入治疗核性白内障患者疗效 被引量:1

Observation of the curative effect of 2%hydroxypropyl methylcellulose pretreatment assisted micro-incision phacoemulsification and intraocular lens implantation in the treatment of patients with nuclear cataract

摘要 目的:探究2%羟丙基甲基纤维素预处理辅助微切口超声乳化吸除术、人工晶状体植入治疗核性白内障患者疗效。方法:选取2017年5月至2019年4月核性白内障患者74例,行微切口超声乳化吸除术、人工晶状体植入术;根据治疗方案分为对照组和观察组每组各37例患者;对照组患者术中应用平衡盐溶液(BSS),观察组患者应用2%羟丙基甲基纤维素预处理;对比两组患者的手术时间、药效持续时间、术中药物用量、术中药物应用次数、术后视力恢复情况及术前、术后1 d角膜荧光素染色、下睑中央泪河高度(TMH)、泪膜破裂时间(BUT)。结果:观察组患者的药效持续时间为(19.38±1.25)min,较对照组患者的(1.97±0.46)min长,观察组患者的术中药物用量为(0.19±0.08)mL、应用次数(1.06±0.38)次,较对照组患者的术中用药量(8.92±0.47)mL、(13.85±2.76)次少(P<0.05);术后1 d观察组患者的角膜荧光素染色(4.54±3.38)分,低于对照组患者的(7.83±3.29)分,观察组患者的TMH值为(556.75±65.29)μm、BUT值为(5.71±2.24)s,高于对照组患者的(487.53±53.28)μm和(3.28±2.06)s(P<0.05);观察组术后1周和术后2周视力达标率分别为75.68%和89.19%,较对照组患者的51.35%、和89.19%高(P<0.05)。结论:2%羟丙基甲基纤维素预处理辅助微切口超声乳化吸除术、人工晶状体植入治疗核性白内障患者,可改善术后泪膜稳定性,促进视力恢复。 Objective:To explore the efficacy of 2%hydroxypropyl methylcellulose pretreatment assisted micro-incision phacoemulsification and intraocular lens implantation in the treatment of patients with nuclear cataract.Methods:A total of 74 patients with nuclear cataract from May 2017 to April 2019 were selected for micro-incision phacoemulsification and intraocular lens implantation;according to the treatment plan,they were divided into control group and observation group with 37 patients in each group;patients in the control group were treated with balanced salt solution(BSS),and patients in the observation group were pretreated with 2%hydroxypropyl methylcellulose;the operation time,duration of drug effect,drug dosage,intraoperative drug application times,and postoperative vision recovery,corneal fluorescein staining,central tear river height(TMH)and tear film rupture time(BUT)before and 1 day after surgery of patients of the two groups were compared.Results:The duration of drug effect in the observation group was(19.38±1.25)min longer than that in the control group(1.97±0.46)min,the drug dosage(0.19±0.08)mL and the number of applications(1.06±0.38)times were less than those of the control group[(8.92±0.47)mL,(13.85±2.76)times](P<0.05);the scores of corneal fluorescein staining(4.54±3.38)in the observation group were lower than those in the control group(7.83±3.29)at 1 day after operation,TMH(556.75±65.29)μm and BUT(5.71±2.24)s were higher than the control group[(487.53±53.28)μm,(3.28±2.06)s](P<0.05);the visual acuity attainment rate(75.68%,89.19%)of the observation group at 1 week and 2 weeks after operation was higher than that of the control group(51.35%,89.19%)(P<0.05).Conclusion:2%hydroxypropyl methylcellulose pretreatment assisted micro-incision phacoemulsification and intraocular lens implantation for the treatment of patients with nuclear cataract can improve the stability of the tear film and promote the recovery of vision.
