摘要
目的观察重复经颅磁刺激(rTMS)联合抗抑郁剂舍曲林对难治性躯体化障碍患者的增效作用。方法选择绍兴市第七人民医院2015年12月~2017年12月心身障碍病区住院的79例躯体化障碍患者作为研究对象,随机分为研究组(rTMS+舍曲林)40例、对照组(舍曲林)39例,治疗8周。两组病例在治疗前和治疗后的第2、4、6、8周末,分别测评患者健康问卷躯体症状群量表(PHQ-15)、治疗中需处理的不良反应量表(TESS)。疗效评价以PHQ-15减分率为标准,不良反应以TESS评分为标准。结果两组患者各时期PHQ-15评分组间比较,在第4、6、8周末差异均有统计学意义(P<0.05),两组疗效比较差异有统计学意义(P<0.05)。各时期TESS评分组间比较差异均无统计学意义(P>0.05)。结论rTMS联合舍曲林治疗难治性躯体化障碍患者具有增效作用,不明显增加患者的不良反应。
Objective To observe the synergistic effect of repetitive transcranial magnetic stimulation(rTMS)combined with antidepressant sertraline on patients with refractory somatoform disorders.Methods Seventy-nine patients with somatization disorders in the psychosomatic disorder ward from December 2015 to December 2017 in the Seventh People's Hospital of Shaoxing were randomly divided into study group(rTMS+sertraline)with 40 cases,and control group(sertraline)with 39 cases.They were treated for 8 weeks.The Health Questionnaire Somatic Symptoms Scale(PHQ-15)and the adverse reaction scale(TESS)to be treated during the treatment between two groups were evaluated before treatment,and at the 2nd,4th,6th,and 8th week after treatment.The efficacy evaluation was based on the PHQ-15 reduction rate,and the adverse reaction was based on the TESS score.Results There were significant differences between the two groups in the PHQ-15 scores at the 4th,6th,and 8th week(P<0.05).The difference in the efficacy between the two groups was statistically significant(P<0.05).There was no significant difference in the TESS scores between the two groups(P>0.05).Conclusion rTMS combined with sertraline has a synergistic effect in the treatment of patients with refractory somatization disorders,and does not significantly increase the adverse reactions of patients.
