3D打印辅助口内入路精准治疗下颌骨粉碎性骨折 被引量:7

Accurate treatment of mandibular comminuted fracture by intraoral approach assisted by 3D printing

摘要 目的探讨3D打印技术及口内切口在下颌骨粉碎性骨折中的应用价值。方法选取我科2015年1月至2018年6月诊治的41例下颌骨粉碎性骨折患者,根据是否应用3D打印技术分为3D组和对照组。3D组术前采用三维虚拟复位,3D打印复位后的骨折模型,重建钛板预弯制成形,术中采用口内改良切口及成形钛板坚强内固定下颌骨粉碎性骨折;对照组采用传统CT平扫影像资料制订手术方案。比较两组的手术时间、术中出血量,术后手术切口愈合效果、是否伴有面瘫以及双侧下颌骨的对称性。结果3D组手术时间、术中出血量、术后面型、双侧下颌骨的对称性及有效率优于对照组,两组相比差异有统计学意义(P<0.05);两组术后面瘫发生的差异无统计学意义(P>0.05)。结论对于粉碎性下颌骨骨折,采用三维虚拟复位、3D打印及口内改良切口重建钛板坚强内固定的修复方式,可以减少术中操作时间及出血量,实现面部无瘢痕、有效恢复面部外形及咬合功能的手术效果。 Objective To explore the application value of 3 D printing technology and intraoral incision incomminuted fracture of mandible.Methods41 patients with comminuted fracture of mandible from January 2015 toJune 2018.They were divided into 3 D group and conventional group according to whether they received 3 D printing.3 D virtual reseted and printed fracture model,and titanium plate was prebended in,modified incision and titaniumplate were used to fix the comminuted fracture of mandible in 3 D group.In the conventional group,conventional CTplain scan image data were used to formulate the operation plan.The operation time,bleeding volume,facialparalysis and symmetry of mandible were compared.ResultsThe operation time,intraoperative bleeding volume,postoperative type,bilateral mandible symmetry and efficiency of 3 D group were better than those of the conventionalgroup,the difference was statistically significant(P<0.05).ConclusionFor comminuted mandible fracture,3 Dvirtual reduction,3 D printing and reconstruction of titanium plate with improved incision in mouth can reduce theoperation time and the amount of bleeding during the operation,and achieve the surgical effect of no scar on theface,effective recovery of the facial shape and occlusion function.
