加味参术扶正祛邪方对早期食管癌术后患者血浆TGF-β1、NGAL水平及预后的影响 被引量:2

Effect of Modified Canshu Fuzheng Quxie Recipe on Levels of Plasma TGF-β1 and NGAL and Prognosis for Pa-tients after Early Esophageal Cancer Surgery

摘要 目的:观察加味参术扶正祛邪方对早期食管癌术后患者血浆转化生长因子-β1(transforming growth factor-β1,TGF-β1)、中性粒细胞明胶酶相关脂质运载蛋白(Neutrophil Gelatinaseassociated Lipocalin,NGAL)水平以及预后的影响。方法:采用随机数表发将本院于2017年12月至2019年12月收治的早期行食管癌手术的96例患者分为对照组和观察组,各48例。2组患者术后均接受化疗,观察组在此基础上给予加味参术扶正祛邪方中药内服,2组均治疗2个疗程。对比分析两组的临床疗效、治疗前后T淋巴细胞亚群(CD3^+、CD4^+、CD8^+)和免疫球蛋白(lgG、lgA、lgM)含量变化情况、血浆TGF-β1、NGAL水平,并对患者出院后进行1年的随访,观察预后。结果:两组的临床治疗有效率分别为93.75%和79.17%,观察组明显优于对照组(P<0.05)。观察组治疗后的T淋巴细胞亚群指标CD3^+、CD4^+以及CD4^+/CD8^+均显著高于对照组(P<0.05)。治疗后,观察组患者的lgG、lgA水平显著高于对照组(P<0.05)。两组治疗后的TGF-β1、NGAL水平较治疗前明显降低(P<0.05),且观察组降低幅度大于对照组(P<0.05)。观察组1年后复发率显著低于对照组(P<0.05)。结论:将加味参术扶正祛邪方应用于早期食管癌术后患者中,能降低患者血浆TGF-β1、NGAL水平,提高其免疫力,效果确切。 Objective:To explore the effect of Modified Canshu Fuzheng Quxie Recipe on levels of transforming growth factor-β1(TGF-β1) and neutrophil gelatinase-associated lipocalin(NGAL),and prognosis for patients after early esophageal cancer surgery.Methods:A total of 96 patients who underwent early esophageal cancer surgery and were admitted to the hospital from December 2016 to December 2019 were divided into controlled group and observation group by the random number table method,48 cases in each group.Both groups underwent chemotherapy after surgery.On this basis,the observation group was given Modified Canshu Fuzheng Quxie Recipe orally.Both groups were treated for 2 courses.The clinical curative effect,changes in contents of T lymphocyte subsets(CD3+,CD4+,CD8+) and immunoglobulin(lgG,lgA,lgM) before and after treatment,levels of plasma TGF-β1 and NGAL were compared between the two groups.Patients were followed up for 1 year after discharge to observe their prognosis.Results:The response rate of clinical treatment in the observation group was significantly better than that in the controlled group(93.75% vs 79.17%)(P<0.05).After treatment,T3 lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) in the observation group were significantly higher than those in the controlled group(P<0.05).After treatment,levels of lgG and lgA in the observation group were significantly higher than those in the controlled group(P<0.05).After treatment,levels of TGF-β1 and NGAL in both groups were significantly decreased(P<0.05),whose decrease amplitude in the observation group was greater than that in the controlled group(P<0.05).The recurrence rate in the observation group one year later was significantly lower than that in the controlled group(P<0.05).Conclusion:The application of Modified Canshu Fuzheng Quxie Recipe in patients after early esophageal cancer surgery can reduce levels of plasma TGF-β1 and NGAL,improve immunity,with significant effect.
