摘要
目的:观察颈三针联合穴位贴敷治疗神经根型颈椎病患者的疗效及对颈部生物学相关参数、血清TNF-α水平的影响。方法:选取342例神经根型颈椎病患者进行随机分组,分别为观察组(n=171)和对照组(n=171),对照组给予颈舒颗粒治疗,观察组在对照组治疗基础上给予颈三针联合穴位贴敷治疗,观察两组治疗前及治疗30d后的中医症状积分、临床疗效、治疗前及治疗后的颈椎生物学相关参数测定和治疗前及治疗30d后的血清肿瘤坏死因子-α水平。结果:治疗后,两组眩晕头痛、目眩、肢体麻木、上肢刺痛的症状积分显著降低(P<0.05),观察组下降幅度显著小于对照组(P<0.05);组间比较临床总有效率发现,观察组(95.91%)显著高于对照组(85.38%)(P<0.05);治疗后,两组颈椎活动度(左旋转、右旋转、左侧屈、右侧屈共4个方向)、生理曲度和颈椎肌力(前屈、后伸、左侧屈及右侧屈肌力共4个方向)测定值显著提高(P<0.05),观察组变化幅度显著大于对照组(P<0.05)。治疗后,两组血清TNF-α水平显著降低(P<0.05),观察组变化差值显著大于对照组(P<0.05)。结论:颈三针联合穴位贴敷治疗可明显改善神经根型颈椎病患者颈功能活动度和生理曲度,缓解颈部肌肉群紧张、痉挛症状,缓解疼痛症状,临床疗效确切。
Objective:To explore the efficacy of cervical three needles combined with acupoint application on patients with cervical spondylotic radiculopathy and its effects on cervical biological-related parameters and serum TNF-α.Methods:342 cases of patients with cervical spondylotic radiculopathy were selected and randomly divided into an observation group(n=171) and a controlled group(n=171).The controlled group was treated with Jingshu Granules,and the observation group was given cervical three needles combined with acupoint application on the basis of the controlled group.The TCM symptoms scores before treatment and after 30 d of treatment,clinical efficacy,cervical biologic parameters before and after treatment,and serum tumor necrosis factor-α level before treatment and after 30 d of treatment were observed in the two groups.Results:After treatment,the scores of symptoms of vertigo headache,dizziness,limb numbness and upper limb tingling were significantly decreased in the two groups(P<0.05),and the decreases in the observation group were significantly lower than those in the controlled group(P<0.05).The total clinical effective rate in the observation group was significantly higher than that in the controlled group(95.91% vs 85.38%)(P<0.05).After treatment,the cervical vertebrae mobility(4 directions of left rotation,right rotation,left lateral flexion,right lateral flexion),physiological curvature and cervical muscle strength(4 directions of anteflexion,rear protraction,left lateral flexion and right lateral flexion strengths) were significantly increased in the two groups(P<0.05),and the changes in the observation group were significantly greater than those in the controlled group(P<0.05).After treatment,the serum TNF-α level was significantly decreased in the two groups(P<0.05),and the difference of change in the observation group was significantly greater than that in the controlled group(P<0.05).Conclusion:Cervical three needles combined with acupoint application can significantly improve the cervical function mobility and physiological curvature of patients with cervical spondylotic radiculopathy,and relieve the tension of neck muscles,the spasm symptoms and the pain symptoms,and it has exact clinical efficacy.
