摘要
目的研究2018年宁波市北仑区呼吸道非结核分枝杆菌(NTM)的分离率、菌种分布、耐药率等现状。方法收集2018年1月-12月疑似肺结核患者的痰液标本或肺泡灌洗液1 085份进行分枝杆菌培养,分枝杆菌阳性菌且胶体金阴性菌株进一步采用晶芯基因芯片进行菌种鉴定,对NTM菌株采用微孔板药敏试验。结果 NTM分离率为15.4%(41/267),NTM菌种类别有5种,前3位菌种分别为胞内分枝杆菌16株(37.2%)、堪萨斯分枝杆菌12株(27.9%)、龟/脓肿分枝杆菌8株(18.6%),未检出混合感染者。RGM耐药率较低的药物为克拉霉素(0.0%)、阿米卡星(37.5%)、加替沙星(37.5%)、莫西沙星(37.5%),SGM中克拉霉素、阿米卡星、利福布丁、莫西沙星耐药率均为0.0%。结论宁波市北仑区优势非结核分枝杆菌群以胞内分枝杆菌和堪萨斯分枝杆菌为主,NTM对经典一线抗结核药物耐药率高,不同种NTM体外药敏结果差异大。
Objective To study the isolation rate,distribution of Nontuberculosis mycobacterium(NTM)and resistance rate from respiratory tract in Beilun District of Ningbo City in 2018.Methods A total of 1085 sputum specimens or alveolar lavage fluids from patients with suspected tuberculosis were collected from January to December 2018 for mycobacterial culture.The mycobacterial-positive bacteria and the colloidal gold-negative strain were further identified by the method of crystal core gene chip.In addition,a microplate sensitivity test of the NTM strain was performed.Results The separation rate of NTM was 15.4%(41/267),and there were 5 categories of NTM.The first three were:16 strains of M.intracellulare(37.2%),12 strains of M.kansasii(27.9%),and 8 strains of M.chelonae/abscessus complex(18.6%),and no mixed infections were detected.The drug with lower resistance rate in RGM were clarithromycin(0.0%),amikacin(37.5%),gatifloxacin(37.5%),moxifloxacin(37.5%),and the resistance rate of clarithromycin,amikacin,rifabutin and moxifloxacin in SGM was 0.0%.Conclusion The dominant Nontuberculous mycobacteria in Beilun District is mainly M.intracellular and M.kansasii.Additionally,NTM has a high resistance rate to classic first-line anti-tuberculosis drugs,and different NTM drug susceptibility results vary widely.
