摘要
听力障碍是儿童常见的发育性障碍。目前,人工耳蜗植入是听障儿童听力重建的有效方法之一。人工耳蜗植入儿童的听力语言功能测查及其预后评估主要依赖外周多维度的主客观检测技术和方法,尚缺乏基于脑功能的测评手段。本文回顾了健听儿童听力语言发展和脑发育规律的研究,以及脑影像技术,特别是功能性近红外光谱成像技术(functionalnear-infrared spectroscopy,f NIRS)在听障人群中的相关研究和应用,提出了基于脑功能的人工耳蜗植入儿童听力语言功能测评的构想,为客观了解听障儿童人工耳蜗植入前后的听力语言功能发展状况,评估神经可塑性以及辅助制定康复方案提供了可能。
Hearing disorder is one of the common developmental disorders in China. For some children with severe hearing disorder, cochlear implant(CI) is one of the most effective treatment approaches for now. However, it remains difficult to objectively and efficiently assess the hearing and language functions of these children, as well as their prognosis of rehabilitative treatment. To address this issue, here we reviewed the literatures on normal children’s hearing and language development and the corresponding neural basis such as brain development. We especially focused on the scientific and clinical applications of neuroimaging techniques, functional near-infrared and spectroscopy(fNIRS) in particular, in this field.The potential advantages and disadvantages of each technique were discussed, basing on those we proposed a brain-based approach assessing the hearing and language functions and the rehabilitation potentials for children with CI. The ultimate goal of this review is to provide a framework for the scientific understanding and clinical assessment of the hearing and language development, and to evaluate the rehabilitation programs in these children before and after CI.
