摘要
视网膜静脉阻塞(RVO)是全球第二高发的视网膜血管疾病,常导致视力丧失,影响超过1600万人。中医辨证论治思想在本病的治疗上有其独特之处,中西医联合治疗可缩短本病病程,在促进黄斑水肿减退、视功能恢复、全身情况改善等方面较单纯西医治疗优势明显。在前人认识以及笔者30余年临床经验的基础上,结合RVO中西医发病机理,高度概括本病病机为“络损积阻”,治疗上主张祛积通络法,并自拟“祛积通络方”。这与西医中改善血流动力学、改善与修复微循环障碍的思路一致。前期的研究发现,祛积通络方在减轻黄斑水肿和促进视网膜出血吸收,防止再出血方面有较好疗效。本文主要从当前RVO西医、中医诊治的现状,以及从中医络损积阻的认识及祛积通络的临床实践中的一些认识和体会进行深入阐述。
Retinal vein occlusion(RVO)is the second most prevalent retinal vascular disease in the world,which often causes vision loss,and affects more than 16 million people.Traditional Chinese Medicine has its unique features in the treatment of this disease.It has obvious advantages over simple Western Medicine treatment in promoting macular edema,visual function recovery,and improvement of general conditions.Based on the predecessors'knowledge and more than 30 years of clinical experience,combined with the pathogenesis of RVO in Traditional Chinese and Western Medicine,the author highly summarized the core pathogenesis of this disease as collateral damage and accumulation obstruction,and advocated the principle of Quji Tongluo,which was consistent with the ideas of Western Medicine in improving hemodynamics,improving and repairing microcirculation obstacles,and drew up Quji Tongluo prescription.Previous studies found that Quji Tongluo prescription had a good effect in reducing macular edema,promoting the absorption of retinal hemorrhage and preventing rebleeding.This article mainly discussed the current status of the diagnosis and treatment of RVO in Western Medicine and Traditional Chinese Medicine,as well as the understanding of Traditional Chinese Medicine on collateral damage and accumulation obstruction,and some knowledge and experience of Quji Tongluo method in clinical practice.
