中心静脉血氧饱和度联合中心静脉——动脉血二氧化碳分压差在感染性休克的意义 被引量:3

Significance of differential partial Pressure of Carbon Dioxide in arterial Blood between central venous Oxygenation and central venous System in septic Shock

摘要 目的研究中心静脉血氧饱和度联合中心静脉—动脉血二氧化碳分压差在指导感染性休克患者液体复苏的意义。方法对长沙市第三医院2017年4月—2019年4月期间治疗的70例感染性休克患者进行研究,按照随机数字表法分为两组,各35例,均在常规治疗的基础上给予体液复苏,对照组患者复苏目标为ScvO2(中心血氧饱和度)≥0.70,治疗组患者在对照组控制目标的基础上联合Pcv-aCO2<6 mmHg,观察两组患者治疗前后各项情况、治疗转归。结果治疗前两组患者各项检测数据之间无显著差异(P>0.05);治疗后研究组CVP、BE、LAC、MAP优于对照组,差异显著(P<0.05);研究组6 h平均补液量、ICU入住时间、机械通气时间上均优于对照组,差异显著有统计学意义(P<0.05)。结论对于感染性休克患者,实施中心静脉血氧饱和度联合中心静脉—动脉血二氧化碳分压差,能够改善患者循环血流量,使得患者缺氧情况有效好转,减少患者机械通气时间。 Objective To study the significance of the difference of partial pressure of carbon dioxide in the blood of central vein oxygenation combined with central vein as a guide for fluid resuscitation in septic shock patients.Methods A total of 70 patients with septic shock who were treated in the third hospital of Changsha from April 2017 to April 2019 were divided into two groups according to the random number table,35 patients in each group were given fluid resuscitation on the basis of routine treatment,patients in the control group were resuscitated with SCVO2(central oxygenation)≥0.70.Patients in the treatment group were observed before and after treatment with Pcv-aCO2<6 mmHg on the basis of the control group.The conditions and outcomes of the two groups were observed before and after treatment.Results There was no significant difference between the two groups before treatment(P>0.05);after treatment,CVP,BE,LAC and MAP in the study group were better than those in the control group(P<0.05);The average volume of fluid replacement,ICU stay time and mechanical ventilation time in the study group were better than those in the control group(P<0.05).Conclusion For patients with septic shock,the implementation of central venous oxygenation combined with the difference of partial pressure of carbon dioxide in the arterial blood can improve the circulating blood flow and make the hypoxia situation of patients effectively better,reduce the duration of mechanical ventilation in patients.
