摘要
目的分析克氏针穿越带蒂皮瓣在末节指骨开放性骨折合并软组织缺损中的临床应用。方法选取2017年1月至2018年1月普洱市人民医院收治的末节指骨开放性骨折合并软组织缺损患者48例作为研究对象,按随机数字表法分为对照组和研究组两组,每组24例。对照组患者采用注射针头固定治疗,研究组患者采用克氏针穿越带蒂皮瓣治疗。比较两组患者治疗3个月后的疗效、一般指标及并发症发生。结果研究组患者的治疗优良率为95.83%,显著高于对照组的66.67%(P<0.05)。研究组患者手术时间、术中出血量、住院时间均少于对照组(P<0.05)。研究组患者并发症发生率为8.33%,显著低于对照组的37.50%(P<0.05)。结论克氏针穿越带蒂皮瓣用于治疗末节指骨开放性骨折合并软组织缺损,能够达到更好的临床治疗效果,有效缩短了手术时间,提高了手术安全性,并减少并发症的发生率。
Objective To analyze the clinical application of Kirschner needle crossing pedicled flap in the open fracture of the distal phalanx and soft tissue defect.Methods A total of 48 patients with open fractures of the distal phalanx and soft tissue defect admitted to Pu'er People's Hospital from January 2017 to January 2018 were selected as the study subjects,and randomLy divided into two groups,control gr oup and study group,with 24 patients in each group.The control group was treated with needle fixation and the study group was treated with Kirschner needle through pedicle flap.The curative effect,general index and complications were compared between the two groups after 3 months of treatment.Results Excellent and good rate in the study group was 95.83%,which was significantly higher than that in the control group was 66.67%(P<0.05).All patients in the study group had less operation time,intraoperative bleeding volume and hospitalization time than those in the control group(P<0.05).Complications in the study group were 8.33%,which was significantly lower than that in the control group was 37.50%(P<0.05).Conclusion The treatment of open fracture and soft tissue defect of end phalangeal bone by Kirschner's needle through pedicled flap can achieve better therapeutic effect,shorten operation time,improve operation safety and reduce complications.
