摘要
目的分析原发性肠道T细胞淋巴瘤(PITCL)和B细胞淋巴瘤(PIBCL)的CT小肠造影(CTE)表现,探讨CTE对两者诊断及鉴别诊断的应用价值。方法回顾性分析我院经病理证实且治疗前的23例肠道PITCL及58例PIBCL患者的CTE图像,从病变影像学分型、病灶数量、发生部位、受累肠壁增厚程度、强化方式及并发症几个方面进行比较,分析两种病变CTE征象的差异。结果弥漫性回肠空肠化型为原发性肠道T细胞淋巴瘤所特有,其它分型之间差异无统计学意义。两种疾病的的病灶数量、发生部位、受累肠壁增厚程度、并发症等方面之间比较差异有统计学意义,而强化方式之间差异没有统计学意义。结论CTE能够用于PITCL和PIBCL之间的诊断和鉴别诊断,具有较强的临床实用性,有望代替传统影像学检查方法和临床侵入性检查,成为诊断和鉴别诊断PITCL和PIBCL的主要方法。
Objective To analyze the CT appearances of primary intestinal T-cell lymphoma(PITCL)and B-cell lymphoma(PIBCL),and to explore the value of CTE in the diagnosis and differential diagnosis of PITCL and B-cell lymphoma.Methods CTE images of 23 patients with PITCL and 58 patients with PIBCL confirmed by pathology before treatment in our hospital from 2008 to 2018 were retrospectively analyzed.The difference of CTE signs between the two types of lesions was analyzed by comparing the imaging types,number of lesions,location,thickness of the involved intestinal wall,enhancement methods and complications.Results Diffusible jejunum mucosa ileum metaplasia type was unique to primary intestinal T-cell lymphoma,and there was no significant difference between other types.There were significant differences in the number of lesions,site of occurrence,thickness of involved intestinal wall and complications between PITCL and PIBCL,however,there was no significant difference between the ways of reinforcement.Conclusion CTE can be used in the diagnosis and differential diagnosis between PITCL and PIBCL,and has strong clinical practicability.It is expected to replace the traditional imaging methods and clinical invasive examinations and become the main method for the diagnosis and differential diagnosis of PITCL and PIBCL.
