摘要 目的探讨3D打印技术辅助On-table技术手术治疗MasonⅢ型桡骨头骨折的临床疗效。方法回顾性分析自2018-01—2018-06诊治的21例MasonⅢ型桡骨头骨折,10例采用3D打印技术辅助On-table技术手术治疗(观察组),11例采用单纯On-table技术治疗(对照组)。比较2组手术时间、术中出血量、术后6个月Mayo肘关节功能评分、肘关节屈曲及背伸活动度。结果21例均获得随访,随访时间平均10.9(6~18)个月。对照组出现2例肘关节僵硬,对症治疗后好转。观察组手术时间、术中出血量较对照组少,差异有统计学意义(P<0.05)。术后6个月观察组Mayo肘关节功能评分较对照组高,屈曲活动度较对照组高,背伸活动度较对照组低,差异有统计学意义(P<0.05)。结论3D打印技术辅助Ontable技术手术治疗MasonⅢ型桡骨头骨折在术前制定了个性化的手术方案,有效减少了手术时间和术中出血量,术后肘关节功能恢复满意。 Objective To explore the clinical efficacy of 3D printing assisted On-table reconstruction in the treatment of Mason typeⅢradial head fractures.Methods A retrospective study was performed on 21 patients with Mason typeⅢradial head fractures from January 2018 to June 2018.Ten cases underwent On-table reconstruction assisted by 3D printing technology(observation group)and 11 cases underwent On-table reconstruction(control group).The operation time and intraoperative blood loss as well as the Mayo score,the flexion and extension angle of elbow joint 6 months after operation were compared between the two groups.Results All 21 cases were followed-up for 6 to 18 months with an average of 10.9 months.There were 2 cases of elbow stiffness in the control group which improved after symptomatic treatment.The operation time and intraoperative blood loss in the observation group were less than those in the control group,with statistically significant differences(P<0.05).Six months after operation,the Mayo score and flexion angle of elbow joint in the observation group were greater than those in the control group,the extension angle of elbow joint in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion The 3D printing technology assisted On-table reconstruction can make an individualized operation plan according to different subjects,shorten the operation time,reduce the amount of bleeding,and get more satisfactory elbow joint kinematics recovery.
