摘要
目的分析抗核抗体(ANA)和ANA谱的相关性,探讨其在临床疾病中的特征.方法采用间接免疫荧光法检测抗核抗体(IgG),免疫印迹法检测抗核抗体谱.结果690例怀疑自身抗体阳性的临床标本涉及多种疾病,其中ANA阳性标本为494例,以滴度为1:1000的阳性标本居多;自身免疫性疾病ANA滴度偏高,其他继发性免疫反应疾病ANA滴度在不同疾病中的分布无明显规律;ANA谱检测特异性自身抗体阳性标本共245例,抗SSA和抗Ro-52抗体阳性率最高;抗核小体、抗dsDNA、抗组蛋白、抗Sm、抗U1-nRNP、抗着丝点B蛋白、抗SSB、抗抗线粒体抗体-M2抗体阳性标本ANA滴度主要集中在1:320~1:1000;抗SSA、抗Ro-52、抗Scl-70、抗Jo-1抗体阳性标本可存在于各种滴度的ANA阳性标本中,无明显的ANA滴度分布特征.结论ANA和ANA谱阳性可出现在各种疾病中,ANA检查对疑似自身抗体阳性患者有重要初筛作用,ANA谱特异性抗体检测对ANA检测阳性的标本具有补充诊断作用,但两种方法检测结果并不完全相关,不能代替,同时检测ANA和ANA谱能更好的为临床诊断和评估预后提供帮助.
Objective To amalyze the stuation and relationship of ANA and ANA profle retspecively,also to explore the feaures of them in clinical disorders.Methods Indirect immunofluorescence was used to detect the antinuclear antibody(IgG),immunoblot assay was used to detect the antinuclear antibody profile.Results 690 samples that suspected auto antibody positive were related to a wide range of diseases.There were totally 494 samples ANA positive,more of them were positive with the titer 1:1000.High titer of ANA was found in autoimmune diseases,there was no significantly regularity of titer in dise ases secondary to immunoreaction.There were 245 samples in ANA profile test for specific autoantibody,anti-SSA antibody and anti-Ro-52 antibody had the most positive percentage.The titer of ANA was focused on 1:320~1:1000 in specific antibody positive samples,these antibodies including anti-Nucleosome antibody,anti-dsDNA antibody,anti-Histone antibody,anti-Sm antibody,anti-U1-nRNP antibody,anti-Cenp B antibody,anti-SSB antibody,anti-AMA-M2 antibody.But the titer of ANA had no obvious characteristics in anti-SSA antibody,anti-Ro-52 antibody,anti-Scl-70 antibody and anti-Jo-1 antibody.Conclusion We find that ANA positive or ANA profile positive samples in various disorders.Test for ANA has important role of screening in suspected auto antibody positive patients,test for ANA profile has crucial role of supplementing in ANA positive patients.The results of these two methods are not entirely relevant,and can not be replaced.Simultancously detecting ANA and ama spectrum can provide better help for clinical diagnosis and prognosis evaluaion.
