摘要
目的探讨细胞因子、降钙素原(PCT)、淀粉样蛋白A(SAA)在慢性阻塞性肺病急性加重期(AECOPD)的价值。方法选取2018年8月至2019年3月344例AECOPD患者为观察组,选取同期38例健康者为对照组,分别检测两组细胞因子[白介素-2(IL-2)、白介素-4(IL-4)、白介素-6(IL-6)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)]、PCT和SAA水平并进行分析。结果AECOPD组IL-2、IL-4、TNF-α、IFN-γ水平与对照组比较,差异无统计学意义(P>0.05)。AECOPD组IL-6、IL-10、PCT、SAA水平高于对照组,差异有统计学意义(P<0.05),此外IL-6、IL-10、PCT、SAA的ROC曲线下面积(AUC)分别为0.669、0.667、0.831、0.780,PCT诊断AECOPD的特异性最高(93.8%)。结论测定IL-6、IL-10、PCT、SAA对AECOPD的诊断有一定价值,有利于临床及早进行干预、治疗,进而提高患者的生活质量。
Objective To investigate the levels of cytokines,procalcitonin(PCT)and amyloid A(SAA)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and their application value.Methods A total of 344 patients diagnosed AECOPD admitted to our hospital from August 2018 to March 2019 were enrolled as the observation group.38 healthy controls were selected as the control group.The level of Cytokines(IL-2,IL-4,IL-6,IL-10,TNF-αand IFN-γ),PCT and SAA were tested respectively in the two groups.Results The levels of IL-2,IL-4,TNF-αand IFN-γin the AECOPD group were not significantly different from those in the control group(P>0.05).The levels of IL-6,IL-10,PCT and SAA in the AECOPD group were higher than those in the control group,and the difference was statistically significant(P<0.05).In addition,the areas under the ROC curve(AUC)of IL-6,IL-10,PCT,and SAA were 0.669,0.667,0.831,and 0.780,and PCT had the highest specificity in the diagnosis of AECOPD(93.8%).Conclusion The determination of IL-6,IL-10,PCT and SAA has certain value in the diagnosis of AECOPD,which is conducive to early intervention and treatment,and thus improve the patients’quality of life.
