摘要
目的探讨中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)及淋巴细胞/单核细胞比值(LMR)在恶性胸腔积液与感染性胸腔积液鉴别诊断中的价值。方法回顾性分析31例恶性胸腔积液及35例感染性胸腔积液患者NLR、PLR、LMR水平,并比较NLR、PLR、LMR水平在两种胸腔积液患者中的差异,并采用受试者工作特征曲线(ROC)分析NLR、PLR、LMR在预测恶性胸腔积液的价值。结果恶性胸腔积液组中NLR及PLR水平显著低于感染性胸腔积液组,LMR水平显著高于感染性胸腔积液组。ROC曲线分析显示NLR、PLR及LMR预测患者为恶性胸腔积液曲线下面积(AUC值)分别为0.85、0.81及0.86,灵敏度分别为65.71%、68.57%、77.14%,特异度分别为93.33%、86.67%、86.67%。结论恶性胸腔积液患者NLR、PLR水平较结核性胸腔积液显著升高,而LMR显著降低,NLR、PLR及LMR在可能在临床鉴别恶性胸腔积液和感染性胸腔积液中发挥重要的价值。
Objective To investigate the value of neutrophil-lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR)and lymphocyte-to-monocyte ratio(LMR)in differential diagnosis of pleural effusion.Methods 31 patients with malignant pleural effusion and 35 patients with infectious pleural effusion were included in the retrospective study.NLR,PLR,and LMR were compared between malignant pleural effusion and infectious pleural effusion patients.Receiver operating characteristic(ROC)curve was used to explore the predictive value of NLR,PLR,and LMR in malignant pleural effusion diagnosis.Results NLR and PLR were lower in malignant pleural effusion patients than in infectious pleural effusion patients.LMR was higher in malignant pleural effusion patients than in infectious pleural effusion patients.The area-under-curve(AUC)values of NLR,PLR and LMR in predicting malignant pleural effusion were 0.85,0.81 and 0.84,respectively.The sensitivity of NLR,PLR and LMR in predicting malignant pleural effusion was 65.71%,68.57%and 77.14%,respectively,and their specificity was 93.33%,86.67%and 86.67%,respectively.Conclusion The difference in NLR,PLR and LMR between malignant pleural effusion patients and infectious pleural effusion patients is significant.NLR,PLR and LMR can be used to differentiate malignant pleural effusion patients and infectious pleural effusion patients.
