摘要
目的分析冠心病患者中不同斑块的分布特点及血清白介素(IL-6)和肿瘤坏死因子(TNF-α)水平。方法回顾分析本院2017年6月至2019年2月收治的90例冠心病患者的临床资料。观察MSCT检查的影像学表现及血清IL-6和TNF-α检查结果进行分析诊断;分析ACS组和SAP组中不同斑块的分布特点;比较不同斑块组和不同冠心病组的血清IL-6和TNF-α水平。结果正常组的血清IL-6和TNF-α水平显著低于软斑块组、混合斑块组和钙化斑块组;混合斑块组与软斑块组的血清IL-6和TNF-α水平比较(P>0.05);软斑块组与钙化斑块组的血清IL-6和TNF-α水平比较(P<0.05);SAP组软斑块和混合斑块的分布显著低于ACS组(P<0.05),但ACS组钙化斑块的分布显著低于SAP组(P<0.05);正常对照组的血清IL-6和TNF-α水平显著低于ACS组和SAP组(P<0.05);SAP组的血清IL-6和TNF-α水平显著低于ACS组(P<0.05)。结论MSCT检查、血清IL-6和TNF-α水平均可有效提示冠状动脉斑块病变的性质,三者联合可作为判断冠状动脉斑块不稳定性的预测指标。
Objective To analyze the distribution characteristics of different plaques and serum interleukin(IL-6)and tumor necrosis factor(TNF-α)levels in patients with coronary heart disease.Methods The clinical data of 90 patients with coronary heart disease admitted to our hospital from June 2017 to February 2019 were analyzed retrospectively.The imaging features of MSCT examination and the results of serum IL-6 and TNF-αexamination were observed and analyzed.The distribution characteristics of different plaques in ACS group and SAP group were analyzed.Serum IL-6 and TNF-αlevels were compared in different plaque groups and different coronary heart disease groups.Results Serum IL-6 and TNF-αlevels were significantly lower in the normal group than those in the soft plaque group,mixed plaque group,and calcified plaque group.The levels of serum IL-6 and TNF-αin the mixed plaque group and the soft plaque group were compared(P>0.05).Serum IL-6 and TNF-αlevels were compared between the soft plaque group and the calcified plaque group(P<0.05).The distribution of soft plaques and mixed plaques in the SAP group was significantly lower than that in the ACS group(P<0.05),but the distribution of calcified plaques in the ACS group was significantly lower than that in the SAP group(P<0.05).The levels of serum IL-6 and TNF-αin the normal control group were significantly lower than those in the ACS group and the SAP group(P<0.05).The levels of serum IL-6 and TNF-αin the SAP group were significantly lower than those in the ACS group(P<0.05).Conclusion MSCT examination,serum IL-6 and TNF-αlevels help to indicate the nature of coronary plaque lesions,and the combination of the three can be used as a predictor of coronary artery plaque instability.
