摘要
目的分析快速康复外科理念(enhanced recovery aftersurgery,ERAS)在老年腹腔镜胆道手术患者麻醉管理中的应用。方法选择我院2018年2月~2019年7月收治的116例老年腹腔镜胆道手术患者为研究对象,依据麻醉护理方法不同分为对照组和观察组。对照组予以常规麻醉护理,观察组给予ERAS麻醉护理干预,比较两组麻醉苏醒时间、住院时间、麻醉苏醒期躁动发生率、麻醉后认知功能障碍发生率、麻醉前后负性情绪变化。结果观察组麻醉苏醒时间、住院时间均短于对照组(P<0.05),麻醉苏醒期躁动发生率、麻醉后认知功能障碍发生率均低于对照组(P<0.05)。两组患者麻醉后的焦虑、抑郁评分比较,观察组低于对照组,差异有统计学意义(均P<0.05)。结论基于快速康复理念的麻醉护理干预用于老年腹腔镜胆道手术患者的麻醉管理,可保障其麻醉安全性,缩短患者的麻醉苏醒时间和住院时间。
Objective To analyze the application of eras(enhanced recovery after surgery)in anesthesia management of elderly patients with laparoscopic biliary surgery.Methods 116 elderly patients with laparoscopic biliary surgery in our hospital from February 2018 to July 2019 were selected as the research objects.According to the anesthesia nursing methods,they were divided into control group and rapid group,and were given routine anesthesia nursing and eras anesthesia nursing intervention.The anesthesia recovery time,hospitalization time,emergence rate of agitation during anesthesia recovery period,incidence of cognitive dysfunction after anesthesia,and negative emotion changes before and after anesthesia were compared between the two groups.Results The anesthesia recovery time and hospitalization time in the fast group were shorter than those in the control group(P<0.05).The incidence of agitation and cognitive dysfunction after anesthesia in the fast group were lower than those in the control group(P<0.05).The scores of anxiety and depression before and after anesthesia in fast group were lower than those in control group(P<0.05).Conclusion Anesthesia nursing intervention based on the concept of rapid rehabilitation can ensure the safety of anesthesia and shorten the anesthesia recovery time and hospitalization time of elderly patients with laparoscopic biliary surgery.
