血栓弹力图在评价脑出血患者围手术期凝血功能中的应用价值 被引量:4

摘要 目的为了明确脑出血患者围手术期凝血状态变化,观察血栓弹力图在评价脑出血患者围手术期凝血功能时的价值和意义。方法根据随机原则要求在该院2018年1-12月急诊手术治疗的脑出血患者中随机选取80例作为观察组研究对象,从同时间段在该院未行急诊手术治疗的少量脑出血患者中选取80例作为对照组研究对象,对比两组血栓弹力图各指标(凝血反应时间、凝块形成时间、凝固角、最大血凝块强度、凝血综合指数)差异。结果观察组患者的D-二聚体、血浆纤维蛋白原、血细胞凝集块形成速率水平[(0.40±0.13)mg/L、(4.57±0.70)g/L、(68.97±6.75)°]明显高于对照组[(0.26±0.09)mg/L、(0.63±0.81)g/L、(58.71±6.12)°],而凝血反应时间和血细胞凝集块形成时间[(6.00±1.90)、(1.09±0.34)min]则明显低于对照组[(6.81±2.26)、(2.24±0.57)min],组间比较差异有统计学意义(tDD=3.214、tFIB=3.758、tR=3.567、tK=5.081,t血细胞凝集块形成速率=3.884,P<0.05)。结论脑出血患者术前和术后不同时间点的凝血状态不同,围术期行血栓弹力图检查,能够及时发现凝血功能障碍及严重程度,效果显著。 Objective To clarify the perioperative coagulation state changes in patients with cerebral hemorrhage,and observe the value and significance of thrombus elasticity diagram in evaluating perioperative coagulation function in patients with cerebral hemorrhage.Methods According to the random principle,80 patients with cerebral hemorrhage treated by emergency surgery in the hospital from January to December 2018 were random selected as the observation group.From a small amount of cerebral hemorrhage that was not treated by emergency surgery in the hospital at the same time Eighty patients were selected as the research object of the control group,and the differences between the two indexes of thrombus elasticity map(coagulation reaction time,clot formation time,coagulation angle,maximum blood clot strength,and coagulation comprehensive index)were compared between the two groups.Results The rate of D-dimer,plasma fibrinogen,and blood cell agglomerate formation in the observation group was[(0.40±0.13)mg/L,(4.57±0.70)g/L,(68.97±6.75)°]was significantly higher than the control group[(0.26±0.09)mg/L,(0.63±0.81)g/L,(58.71±6.12)°],while the coagulation reaction time and blood cell agglomeration formation time[(6.00±1.90)min,(1.09±0.34)min]were significantly lower than the control group[(6.81±2.26)min,(2.24±0.57)min],the difference between the groups was statistically significant(tDD=3.214,tFIB=3.758,tR=3.567,tK=5.081,themagglutination rate=3.884,P<0.05).Conclusion The coagulation state of patients with cerebral hemorrhage is different before and after operation.Thrombus elasticity examination during the perioperative period can detect the coagulation dysfunction and severity in time.
