无创呼吸通气治疗急性高原肺水肿临床研究

摘要 目的通过随机分析无创呼吸通气(NIPPV)治疗急性高原肺水肿的临床意义。方法选取2015年10月—2018年4月来该科住院治疗的50例确诊为急性高原肺水肿的患者,在常规治疗的同时,给予无创呼吸通气治疗(NIPPV),作为实验组,并选择同期未接受无创呼吸通气治疗的50例患者为对照组,监测患者血氧饱和度(SpO2)、血气指标(PO2、CO2)、胸部CT、治疗前后呼吸频率(RR)、心率(HR)、B型脑钠肽(BNP)、无创通气时间等。结果实验组46例均2 h症状改善,4 h肺部啰音消失,47例患者均成功脱机。3例因病情加重,经无创治疗后,转ICU行有创通气治疗;1例因合并脑水肿转上级医院诊治。实验组各项指标优于对照组,差异有统计学意义(P<0.05)。结论急性高原肺水肿,在常规治疗的同时,给予无创呼吸通气(NNPCV),BIPP模式能显著改善低氣血症及低氧血症引起的一系列心脑重要器官的损害。 Objective To randomly analyze the clinical significance of non-invasive respiratory ventilation(NIPPV)in the treatment of acute high-altitude pulmonary edema.Methods Select 50 cases of patients diagnosed with acute high-altitude pulmonary edema from the undergraduate hospitalization in October 2015 to April 2018.At the same time of routine treatment,they will be given non-invasive respiratory ventilation therapy(NIPPV)as the experimental group;and choose not to receive non-invasive respiratory ventilation during the same period 50 casea as the control group.Monitor the patient's blood oxygen saturation(SpO2),blood gas indicators(PO2,CO2),chest CT,respiratory rate before and after treatment(RR),heart rate(HR),type B brain natriuretic peptide(BNP),non-invasive ventilation time,etc.Results All 46 patients in the experimental group improved their symptoms at 2 h,the rales in the lungs disappeared at 4 h,and 47 patients successfully went offline.Three patients were aggravated by the disease and transferred to ICU for invasive ventilation after non-invasive treatment;1 patient was transferred to a high-altitude hospital for treatment due to combined cerebral edema.Each index of the experimental group was better than the control group,the difference was statistically significan(P<0.05).Conclusion Acute high-altitude pulmonary edema,in addition to conventional treatment,given non-invasive respiratory ventilation(NNPCV),BIPP mode can significantly improve a series of important organ damage of heart and brain caused by hypoxemia and hypoxemia.
