摘要
目的探讨价值医疗下分段式护理路径在大隐静脉曲张整合式日间手术患者的应用效果。方法选取2019年1月至12月的大隐静脉曲张整合式日间手术患者为研究对象,随机分为对照组和试验组,对照组43例,试验组59例。对照组进行常规护理路径管理,试验组进采用分段式护理路径管理,比较2组患者的术前准备完成率、手术变异率、术前焦虑评分及出院准备度评分。结果试验组患者在8:30前入院率为92%(54/59),术前血液标本送检9:30前送检率47%(28/59)均高于对照组的47%(20/43)和26%(11/43);试验组术前准备完成率83%(49/59)高于对照组60%(26/43);差异均有统计学意义(P<0.05)。试验组术前焦虑评分(51±5)低于对照组(60±7),而试验组术后出院准备度评分(90±7)高于对照组(79±8),差异均有统计学意义(P<0.05)。结论价值医疗下分段式临床护理路径可提高手术前准备度及出院准备度,减轻患者的经济负担,促进整合式日间手术顺利开展。
Objective To explore the effect of value based segmented nursing pathway in patients with integrated daytime operation of great saphenous vein varicosity.Methods Patients undergoing integrated daytime surgery for great saphenous vein varicosity from January to December 2019 were selected as subjects and were randomly divided into control group and experimental group.There were 43 cases in the control group and 59 cases in the experimental group.The control group was managed by routine nursing pathway.The experimental group adopted segmented nursing pathway management.Preoperative readiness completion rate,surgical variation rate,preoperative anxiety score and discharge readiness score were compared between the two groups.Results The admission rate of patients in the experimental group before 8:30 was 92%(54/59).The submission rate of preoperative blood samples before 9:30 was 47%(28/59),which were higher than those of the control group[47%(20/43)and 26%(11/43)].The completion rate of preoperative preparation in the experimental group was 83%(49/59),higher than that in the control group[60%(26/43)].All the differences were statistically significant(P<0.05).The preoperative anxiety score of the experimental group was(51±5)lower than that(60±7)of the control group.The postoperative discharge readiness score of the experimental group was 90±7,higher than that of the control group(79±8).All the differences were statistically significant(P<0.05).Conclusion The value based segmental clinical nursing pathway can improve the preparation before operation and the preparation for discharge,reduce the economic burden of patients,and promote the smooth development of integrated daytime surgery.
