摘要
目的:探讨Philos钢板与Multiloc髓内钉治疗肱骨近端骨折的临床疗效。方法:选择鹤壁煤业公司总医院于2017年1月至2019年8月收治的肱骨近端骨折患者68例,依据手术方法分为A组与B组,每组34例。A组患者采用Philos钢板治疗,B组患者采用Multiloc髓内钉治疗。比较两组手术指标和术后并发症发生情况,比较两组术后7 d、3个月、6个月肩关节疼痛视觉模拟量表(VAS)评分,术后3、6个月健康指数量表(EQ-5D)指数变化情况,及术后6个月肩关节功能疗效情况。结果:两组手术耗时、术中出血量及术后并发症比较差异未见统计学意义(P>0.05)。两组术后7 d VAS评分比较差异未见统计学意义(P>0.05);B组术后3、6个月VAS评分低于A组(P<0.05)。B组术后3、6个月EQ-5D健康指数高于A组(P<0.05)。B组肩关节功能改善优良率(97.06%,33/34)高于A组(76.47%,26/34),P<0.05。结论:Multiloc髓内钉治疗肱骨近端骨折近期效果与Phlios钢板相当,而远期疗效优于Phlios钢板。
Objective To investigate the effects of Philos plate and Multiloc intramedullary nail in the treatment of proximal humeral fracture.Methods A total of 68 patients with proximal humeral fracture admitted to Hebi Coal Company General Hospital from January 2017 to August 2019 were selected,and they were divided into group A and group B according to the operation methods,with 34 cases in each group.Group A was treated by Philos plate,and group B treated by Multiloc intramedullary nail.The operation indicators,operation indicators,postoperative complications,visual analogue score(VAS)of shoulder pain 7 days,3 months and 6 months after operation,EuroQol Group Five-Dimensional questionnaire(EQ-5D)indexes 3 and 6 months after operation,and shoulder function 6 months after operation were compared between the two groups.Results There was no significant difference between the two groups in operation time,bleeding volume or postoperative complications(P>0.05).There was no significant difference in VAS score between the two groups 7 days after operation(P>0.05),and the VAS score of group B were lower than those of group A 3 and 6 months after operation(P<0.05).The EQ-5D index of group B were higher than those of group A 3 and 6 months after operation(P<0.05).The excellent improvement rate of shoulder function in group B(97.06%,33/34)was higher than that in group A(76.47%,26/34),P<0.05.Conclusions The short-term effects of Multiloc intramedullary nail in the treatment of proximal humeral fracture is similar to that of Philos plate,while the long-term effect is better than that of Philos plate.
