摘要
目的分析青年艾滋病病毒(HIV)感染患者流行特征及健康相关生命质量的影响因素。方法抽取义乌市中心医院免疫力低下门诊和住院治疗、咨询的90例青年HIV感染患者,通过一般情况调查问卷及简体中文版HIV感染者医疗结局健康调查量表(medical outcomes study-HIV health survey,MOS-HIV)调查和分析其流行特征及MOS-HIV各维度评分的影响因素。结果90例患者问卷有效率100%。患者以未婚男性居多,在校学生占比近30%;受教育程度参差不齐;HIV感染年限较短;大多数接受了抗病毒治疗,且达到了病毒抑制。单因素及多因素Logistic回归分析显示,性别为心理健康、健康压力维度评分的独立影响因素(P<0.05);年龄、在校情况、受教育程度、HIV感染年限、抗病毒治疗及病毒抑制情况均为精力/劳累、健康压力、生活状况维度评分的独立影响因素(P<0.05),其中在校情况、受教育程度、HIV感染年限、抗病毒治疗及病毒抑制情况还是心理健康维度的独立影响因素(P<0.05)。结论青年HIV感染患者在性别、是否在校、HIV感染年限等方面有一定特征,性别、年龄、在校情况、HIV感染年限、治疗及病毒抑制情况等均可从不同维度影响青年患者健康相关生命质量。
OBJECTIVE To analyze the epidemiological features of human immunodeficiency virus(HIV)infection in young patients and influencing factors for the health-related life quality.METHODS A total of 90 young patients with HIV infection who were treated and consulted in outpatient department and hospitalized in Yiwu Central Hospital were enrolled in the study.The epidemiological features of HIV infection and influencing factors for Medical Outcomes Study-HIV Health Survey(MOS-HIV)scores were surveyed and analyzed by means of general condition questionnaire and MOS-HIV of simplified Chinese version.RESULTS The rate of valid questionnaire of the 90 patients was 100%.The females and unmarried patients were dominant,the in-school students accounted for about 30%and varied in the education level,the years of HIV infection were short,and most of them received the antiviral therapy and achieve the viral suppression.Univariate analysis and multivariate regression analysis showed that gender was an independent influencing factor for mental health and health stress dimension score(P<0.05);age,school status,education level,years of HIV infection,antiviral treatment and viral suppression were the independent influencing factors for the dimensional scores of energy/fatigue,health stress and life status(P<0.05);the school status,education level,years of HIV infection,antiviral treatment and viral suppression were also the independent influencing factors for the dimension of mental health(P<0.05).CONCLUSION The young patients with HIV infection show certain characteristics in terms of gender,school status and years of HIV infection.The treatment and viral suppression can affect the health-related life quality of young patients from different dimensions.
