摘要
目的:评价手法按摩联合体位垫使用对乳腺癌根治术后淋巴水肿的预防效果。方法:选取连云港圣安医院外科2016年1月至2019年10月行乳腺癌扩大根治手术治疗的患者42例,随机分为对照组(n=20)和观察组(n=22)。对照组术后给予常规引流护理和功能锻炼指导,观察组在常规引流基础上应用手法按摩护理配合不同时期和不同角度体位垫促进淋巴引流。对比两组患者手术前后的双侧上臂周径差值、疼痛评分(VAS)、淋巴水肿发生率和生活质量(FACTB)评分。结果:术前两组双侧上臂周径差值比较无统计学差异(P>0.05);术后2周和1个月,两组双侧上臂周径差值及VAS评分均有明显差异,观察组上肢轻度以上水肿发生率及VAS平均评分均明显低于对照组(P<0.05);术后3个月,观察组跟踪FACTB评分明显高于对照组(P<0.05)。结论:对乳腺癌根治术后患者实施手法按摩并辅助使用不同角度体位垫,可明显促进淋巴引流,减少术后淋巴水肿发生,缓解术后疼痛,提高生活质量。
Objective:To evaluate the effect of manual massage combined with position pads on the prevention of lymphedema after radical mastectomy.Methods:Forty-two patients who underwent radical mastectomy for breast cancer from January 2016 to October 2019 in Lianyungang Sheng'an Hospital were randomly divided into the control group(n=20)and the observation group(n=22).The control group was given conventional drainage nursing and functional exercise guidance after operation,and the observation group applied manual massage nursing on the basis of conventional drainage with different periods and different angles of position cushions to promote lymphatic drainage.The bilateral upper arm circumference difference,pain score(VAS),incidence of lymphedema,and quality of life(FACTB)scores were compared between the two groups of patients before and after surgery.Results:There was no significant difference in the difference in peripheral upper arm diameter between the two groups before surgery(P>0.05).At 2 weeks and 1 month after operation,there were significant differences in peripheral upper arm diameter and VAS scores between the two groups.The incidence of mild edema and the average VAS scores were significantly lower than those of the control group(P<0.05);3 months after operation,the follow-up FACTB score of the observation group was significantly higher than that of the control group(P<0.05).Conclusion:Performing manual massage and assisting the use of different angle position pads after radical mastectomy can significantly promote lymphatic drainage,reduce postoperative lymphedema,relieve postoperative pain,and improve quality of life.
