摘要 [目的]分析新疆某三甲医院食管癌患者住院费用结构变化的基本情况及构成,探究导致住院费用持续增长的主要因素,为进一步控制医疗费用不合理增长、优化费用结构提供参考依据。[方法]收集2014-2019年新疆某三甲医院食管癌患者的住院费用资料,采用灰色关联和结构变动度两种方法进行分析。[结果]①食管癌患者住院次均费用呈逐年上升趋势;②药品费的关联系数较大、关联度为0.998、关联序排位第一;③食管癌患者住院费用总结构变动度为25.37%,药品费的结构变动贡献率最大。[结论]药品及耗材费用仍是影响住院费用的主要项目;医护人员医疗服务价值的体现初见成效,但有待进一步提高;推进支付方式改革,控制医疗费用不合理增长。 Objective To analyze basic situation and composition of hospitalization expenses for inpatients with esophageal cancer in a third-class hospital in Xinjiang,explore main factors that leading to continuous increase of hospitalization expenses so as to provide reference for further controlling unreasonable increase of medical expenses and optimizing the cost structure.Methods The hospitalization expenses data of patients with esophageal cancer in a third-class hospital in Xinjiang from 2014 to 2019 were collected and analyzed by grey correlation and structural change methods.Results 1.Per average hospitalization expenses of patients with esophageal cancer showed uptrend year by year.2.The correlation coefficient of drug hospitalization expenses for inpatients with esophageal cancer was 0.998,which in the first of correlation sequence rank.3.Total structural change degree of the summary structure degree of hospital expenses of patients with esophageal cancer was 25.37%,and the contribution rate of structural change in drug cost was the largest.Conclusions The cost of medicine and consumables is still main item that affects the cost of hospitalization.The value of medical service from medical staff is beginning to show results,but it needs to be further improved.The reform of payment mode is promoted to control unreasonable increase of medical expenses.
