摘要
目的探讨血型鉴定及交叉配血对急性白血病贫血患者的检测价值。方法选择我院2018年8月至2019年8月收治的急性白血病贫血患者98例,均接受血型鉴定及交叉配血检测,统计抗体分型、血型鉴定结果及交叉配血结果。结果98例急性白血病贫血患者血液标本中,冷自身抗体、温自身抗体、混合型抗体占比分别为6.12%、84.69%、9.18%。6例冷自身抗体阳性,有特异性自身抗体占33.33%,无特异性自身抗体占66.67%,自身抗体+同种抗体为0。83例温自身抗体阴性,有特异性自身抗体占14.46%,无特异性自身抗体占72.29%,自身抗体+同种抗体占13.25%。9例混合型抗体阳性,有特异性自身抗体为0,无特异性自身抗体占88.89%,自身抗体+同种抗体占11.11%。交叉配血相符与不相符分别占55.10%、44.90%,血型鉴定相符与不相符分别占51.02%、48.98%;ABO血型正、反定型不相符者,三组抗体类型均实施氯喹放散试验,正、反定型均相符。结论血型鉴定及交叉配血对急性白血病贫血患者具有一定的检测价值,可结合氯喹放散试验,以减少输血不良反应的发生。
Objective To investigate the detection value of blood type identification and cross-matching in patients with acute leukemia anemia.Methods Ninety-eight patients with acute leukemia anemia treated in our hospital from August 2018 to August 2019 were selected.All patients were tested by blood type identification and cross-matching,and antibody typing,the results of blood group identification and cross-matching were counted.Results Among the blood samples of 98 patients with acute leukemia anemia,the proportions of cold autoantibodies,warm autoantibodies and mixed antibodies were 6.12%,84.69%and 9.18%,respectively.Six cases of cold autoantibodies were positive,with specific autoantibodies accounted for 33.33%,nonspecific autoantibodies accounted for 66.67%,and autoantibodies+isoantibodies was 0.Eighty-three cases of warm autoantibodies were negative,with specific autoantibodies accounted for 14.46%,nonspecific autoantibodies accounted for 72.29%,and autoantibodies+isoantibodies accounted for 13.25%.Nine cases of mixed antibodies were positive,with specific autoantibodies was 0,nonspecific autoantibodies accounted for 88.89%,and autoantibodies+isoantibodies accounted for 11.11%.Matching and non-matching cross-matching accounted for 55.10%and 44.90%,respectively,matching and non-matching blood group identification accounted for 51.02%,48.98%;for those whose ABO blood type did not match the positive and negative stereotypes,the three groups of antibody types were implemented the chloroquine release test,and the positive and negative stereotypes were consistent.Conclusion Blood group identification and cross-matching have certain detection value for patients with acute leukemia anemia.Chloroquine release test can be combined to reduce the occurrence of adverse reactions of blood transfusion.
