摘要
目的探讨青年肺结核患者支气管肺泡灌洗液(BALF)T淋巴细胞亚群、炎症因子表达情况及检测意义。方法选择2018年1月~2019年11月就诊的青年肺结核患者43例作为青年患病组,选择同期就诊的老年肺结核患者43例作为老年患病组,选择同期体检的健康青年43人作为正常青年组,比较各组BALF中T淋巴细胞亚群、炎症因子水平。结果青年患病组BALF中CD3^+、CD4^+表达水平显著高于老年患病组,NK表达水平显著低于老年患病组(P<0.01);正常青年组BALF中CD3^+、CD4^+表达水平显著高于青年患病组,NK表达水平显著低于青年患病组(P<0.01)。青年患病组BALF中IFN-γ、IL-4、IL-12、IL-10表达水平显著低于老年患病组(P<0.01);正常青年组BALF中IFN-γ、IL-4、IL-12、IL-10表达水平显著低于青年患病组(P<0.01)。正常青年组外周血中IgA表达水平显著低于青年患病组、老年患病组,青年患病组外周血中IgA表达水平显著低于老年患病组(P<0.05)。结论青年肺结核患者BALF中T淋巴细胞免疫功能明显高于老年肺结核患者,但低于健康青年人,炎症反应较老年肺结核患者轻微,但在体液免疫方面无明显差异,检测青年肺结核患者BALF中T淋巴细胞亚群及炎症因子,有利于全面了解患者的细胞免疫功能,评估结核病灶严重程度,为治疗方案选择提供客观依据。
Objective To investigate the expression of T-lymphocyte subsets and inflammatory factors in bronchoalveolar lavage fluid(BALF)of young patients with pulmonary tuberculosis and its significance of detection.Methods 43 young patients with tuberculosis admitted to hospital from January 2018 to November 2019 were selected as young patients group,43 elderly patients with tuberculosis in the same period as old patients group,and 43 healthy young people who came to hospital for physical examination in the same period as normal young people group.And then,the levels of T-lymphocyte subsets and inflammatory factors in BALF of each group were compared.Results The expression levels of CD3^+and CD4^+in BALF of the young patients group were significantly higher than those of the old patients group,but the expression level of NK was significantly lower than that of the old patients group(P<0.01).The expression levels of CD3^+and CD4^+in BALF of the normal young people group were significantly higher than those of the young patients group,but the expression level of NK was significantly lower than that of the young patients group(P<0.01).The expression levels of IFN-γ,IL-4,IL-12 and IL-10 in BALF of the young patients group were significantly lower than those of the old patients group(P<0.01).The expression levels of IFN-γ,IL-4,IL-12 and IL-10 in BALF of the normal young people group were significantly lower than those of the young patients group(P<0.01).The expression level of IgA in the peripheral blood of the normal young people group was significantly lower than that of the young patients group and the old patients group,and the expression level of IgA in the peripheral blood of the young patients group was significantly lower than that of the old patients group(P<0.05).Conclusion The immune function of T-lymphocyte in BALF of young patients with pulmonary tuberculosis is significantly higher than that of old patients,but lower than that of healthy young people,and the inflammatory reaction is milder than that of old patients,but there is no significant difference in humoral immunity.The detection of T-lymphocyte subsets and inflammatory factors in BALF of young patients with pulmonary tuberculosis is conducive to fully understanding the cellular immune function of patients and evaluating the severity of the tuberculosis focus,so as to provide an objective basis for the choice of treatment plan.
