社区中老年人群骨质疏松和低骨量流行情况及影响因素分析 被引量:7

摘要 目的了解苏州市吴江区社区中老年人群骨质疏松和低骨量流行情况并探讨其影响因素。方法采用多阶段分层随机抽样方法,选取吴江区4个乡镇8个村/社区40岁以上居民380人开展问卷调查、身体测量和双能X线骨密度检测,描述分析调查结果和检测结果。结果本次调查骨质疏松检出率为11.32%,低骨量率为43.68%;女性骨质疏松、低骨量率高于男性(P<0.05),年龄组越高,骨质疏松和低骨量率越高。骨质疏松或低骨量组与骨量正常组平均年龄、性别、未接受正规教育者构成、家庭年收入、食用鸡鸭鹅等禽肉情况、食用鲜榨果蔬汁情况、饮用茶情况、身高、体重、腰围、收缩压、空腹血糖、甘油三酯、低密度脂蛋白胆固醇和高密度脂蛋白胆固醇水平差异有统计学意义。多因素logistic回归分析发现,女性、老龄是中老年人群骨质疏松和低骨量发生的危险因素(OR=2.57、3.70,P<0.05),体重增加是保护因素(OR=0.91,P<0.05)。结论吴江区中老年人群低骨量检出率较高,应早期开展筛查和干预,预防中老年人群骨质疏松症和骨折发生。 OBJECTIVE To understand the prevalence of osteoporosis and low bone mass in the elderly population in communities of Wujiang District and explore the influencing factors.METHODS A multi-stage stratified random sampling method was used to select 380 residents over 40 years old from 8 villages/communities in 4 townships in Wujiang District.Questionnaire survey,body measure⁃ment and dual-energy X-ray bone mineral density test were conducted to describe and analyze the results of the survey.RESULTS The detection rate of osteoporosis was 11.32%and the rate of low bone mass was 43.68%.The rate of osteoporosis and low bone mass in female was higher than male(P<0.05).The rates of osteoporosis and low bone mass increased with the increase of the age group.Osteoporosis and low bone mass and bone mass normal group between average age,gender,did not receive formal educators structure,family income,consumption of chickens,ducks and geese and other poultry meat,eat fresh fruit and vegetable juice,drink tea,height,weight,waist circumference,systolic blood pressure,fasting blood sugar,triglyceride,low density lipoprotein cholesterol and high den⁃sity lipoprotein cholesterol level difference were statistically significant.Multivariate Logistic regression analysis found that women and old age were risk factors for osteoporosis and low bone mass(OR=2.57、3.70,P<0.05),and weight gain was a protective factor(OR=0.91,P<0.05).CONCLUSION The detection rate of low bone mass in middle-aged and elderly people in wujiang was high,so early screening and intervention should be carried out to prevent osteoporosis and fracture.
