摘要 目的分析新型冠状病毒肺炎(COVID-19)患者的CT影像特征,为临床实践提供参考。方法计算机检索PubMed、外文医学信息资源检索平台(FMRS)、EMbase、中国知网(CNKI)和万方数据等数据库,搜集报道COVID-19患者CT影像特征的文献,检索时限为2020年1月1日至3月4日。由2名研究员独立筛选文献、收集数据并评价纳入研究的偏倚风险后,采用Stata12.0软件进行Meta分析。结果共纳入20项回顾性研究,共计1982例COVID-19患者。分析结果显示:COVID-19患者肺部病灶多为双肺受累(68.2%,95%CI:61.1%~74.9%)、多肺叶受累(76.7%,95%CI:56.2%~86.2%),15.6%的患者CT无异常表现。病灶密度改变最常见的为磨玻璃样变(69.8%,95%CI:53.1%~84.1%),其他还有实变(42.9%)、空气支气管征(32.9%)和铺路石征(11.2%)。病灶形态有斑片状(40.6%)、条索状(45.2%)、结节状(43.9%)和网格状(29.0%)。部分患者有胸膜肥厚(32.5%),极少数患者合并有胸腔积液(4.2%)。结论COVID-19患者肺部病灶以双肺或多肺叶受累多见,最常见的表现为斑片状、磨玻璃样变,部分患者出现实变、空气支气管征和胸膜肥厚等,胸腔积液少见。 Objective To systematically review the CT imaging features of COVID-19 so as to provide reference for clinical practice.Methods PubMed,EMbase,CNKI and WanFang databases were electronically searched to collect studies about the CT imaging features of COVID-19 from January,2020 to March 4,2020.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies,then,meta-analysis was performed by using Stata12.0 software.Results A total of 20 retrospective studies involving 1982 COVID-19 patients were included.The results of meta-analysis showed that most patients presented bilateral lung(68.2%,95%CI:61.1%-74.9%)or multiple lung lobes(76.7%,95%CI:56.2%-86.2%)lesions,and the ratio of normal CT findings was 15.6%.The most common changes in lesion density were ground-glass opacities(69.8%,95%CI:53.1%-84.1%).Other changes in density included consolidation(42.9%),air bronchogram sign(32.9%)and crazy-paving pattern(11.2%).Patchy(40.6%),cord-like(45.2%),nodular(43.9%)and spider web sign(29.0%)were common lesion shapes in COVID-19 patients.Pleural thickening(32.5%)was found in some patients and pleural effusion(4.2%)was rare.Conclusion The lung lesions of patients with COVID-19 were mostly affected by bilateral or multiple lobes.The most common chest CT findings were patchy and ground-glass opacities.Some patients had consolidation,air bronchogram and pleura hypertrophy,and pleural effusion was not common.
