摘要
目的探讨封闭式负压引流术联合皮瓣移植在慢性创面修复中的临床效果。方法分析2017年6月-2019年10月收治的行慢性创面修复患者84例,根据治疗方式的不同分为研究组(42例)与对照组(42例),两组患者均予传统清创治疗,对照组在此基础上采用转移皮瓣修复创面,研究组在基础治疗上予封闭式负压引流术联合转移皮瓣修复创面,比较两组治疗前与治疗1周后患者疼痛程度、临床恢复指标、治疗效果与治疗期间的不良反应。结果治疗前两组疼痛程度差异无统计学意义,治疗后研究组VSA评分明显低于对照,同时研究组换药次数、创面愈合时间、住院时间均少于对照组(P<0.05);研究组治疗有效率显著高于对照组(P<0.05);研究组治疗期间总不良反应率为7.14%略低于对照组21.43%,但差异无统计学意义(P>0.05)。结论在慢性创面修复中采用封闭式负压引流术联合转移皮瓣修复创面能有效缓解患者疼痛、提高治疗效果,促进患者恢复,且不会增加治疗期间不良反应,安全性高。
Objective To investigate the clinical effect of closed negative pressure drainage combined with flap transplantation in chronic wound repair.Methods A retrospective analysis of our hospital between 2017.6~2019.10,our hospital line of 84 cases of patients with chronic wound repair,according to the different methods of treatment are divided into research group(42 cases)and control group(42 cases),two groups of patients are traditional debridement therapy,the control group on the basis of using transfer skin flap to repair the wound,the team to the closed negative pressure drainage in the treatment of foundation combined transfer of skin flap to repair tissue,compared two groups of patients before and after 1 week treatment pain degree,clinical recovery index,effects and adverse reactions during the treatment.Results Before treatment,there was no significant difference in pain between the two groups.After treatment,the VSA score of the study group was significantly lower than that of the control group.Meanwhile,the number of dressing changes,wound healing time and hospital stay in the study group were all lower than those in the control group(P<0.05).The effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05).The total adverse reaction rate was 7.14%in the study group,slightly lower than 21.43%in the control group,but the difference was not statistically significant(P>0.05).Conclusions Closed negative pressure drainage combined with transfer flap can effectively relieve the pain of patients,improve the therapeutic effect and promote the recovery of patients in chronic wound repair without increasing the adverse reactions during treatment.
