基于脊柱内镜技术的腰椎椎间孔测量及临床意义 被引量:1

LUMBAR INTERVERTEBRAL FORAMEN MEASUREMENT BASED ON THE ENDOSCOPIC SPINE SYSTEM AND ITS CLINICAL SIGNIFICANCE

摘要 目的通过测量椎间孔内椎下切迹的宽度和高度为经皮脊柱内镜手术制定穿刺方案。方法选取行经椎间孔脊柱内镜手术(TESSYS)病人60例,其中25~35岁(A组)、45~55岁(B组)、65~75岁(C组)各20例,男女比例1∶1,每例病人均选取双侧L 4/5椎间孔,共120只椎间孔。各组病人均于术前行腰椎MRI检查,测量椎下切迹高度、宽度。结果A组、B组、C组L 4/5椎间孔椎下切迹宽度分别为(15.14±0.13)、(14.93±0.18)和(14.87±0.19)mm,3组椎下切迹高度分别为(10.16±0.28)、(8.83±0.14)和(9.93±0.15)mm,不同年龄组椎下切迹宽度及高度比较差异有显著性(F=25.30、671.78,P<0.05)。结论术前对椎间孔内神经根走行进行观察可为TESSYS提供影像学参考,并且不同年龄组椎下切迹的高度、宽度与椎体退变有关。 Objective To develop a puncture protocol for the transforaminal endoscopic spine system(TESSYS)by measuring the width and height of the inferior vertebral notch in the intervertebral foramen.Methods A total of 60 patients who were treated with the TESSYS in our hospital were enrolled in this study,including 20 patients aged 25-35 years(group A),20 patients aged 45-55 years(group B),and 20 patients aged 65-75 years(group C).The male-to-female ratio was 1∶1,and the bilateral L 4/5 intervertebral foramens of each patient(a total of 120 intervertebral foramens)were selected in this study.All the patients in each group underwent magnetic resonance imaging(MRI)examination of the lumbar vertebra before surgery to measure the height and width of the inferior vertebral notch.Results The widths of the inferior vertebral notch in the L 4/5 intervertebral foramen in groups A,B,and C were(15.14±0.13)mm,(14.93±0.18)mm,and(14.87±0.19)mm,respectively,and the heights of the inferior vertebral notch in the three groups were(10.16±0.28)mm,(8.83±0.14)mm,and(9.93±0.15)mm,respectively;there were significant differences in the width and height of the inferior vertebral notch between different age groups(F=25.30,671.78;both P<0.05).Conclusion Preoperative observation of the path of exiting nerve roots in the intervertebral foramen can provide an imaging reference for TESSYS,and the differences in height and width of the inferior vertebral notch between different age groups are related to vertebral degeneration.
