摘要
目的为临床合理使用抗抑郁药提供参考。方法采用世界卫生组织(WHO)推荐的限定日剂量(DDD),统计广东地区151家医院2015年至2019年抗抑郁药的主要品种、销售金额、用药频度(DDDs)、日均费用(DDC)及药物利用指数(DUI)。结果抗抑郁药销售金额和DDDs逐年增长;选择性5-羟色胺再摄取抑制剂(SSRIs)及5-羟色胺和去甲肾上腺素再摄取抑制剂(sSNRIs)的销售金额和DDDs构成比均接近80%;5年来销售金额和DDDs均排名前5的药物是艾司西酞普兰、氟哌噻吨美利曲辛、舍曲林、文拉法辛和帕罗西汀;阿戈美拉汀的销售金额增长最快(年均增长率194.75%),DDDs年均增长率207.40%;三环类抗抑郁药(TCAs)使用极少,销售金额和DDDs均排名靠后;各年度DUI总体趋近1;新型抗抑郁药DDC较高,但呈逐年下降趋势。结论广东地区抗抑郁药的销售金额和DDDs呈逐年增长趋势;以SSRIs和sSNRIs为代表的新型抗抑郁药已替代传统的抗抑郁药,占主导地位。
Objective To provide evidence for rational use of antidepressants.Methods The defined daily dose(DDD)analytic method recommended by the World Health Organization(WHO)was adopted to perform the statistical analysis on the consumption sum of antidepressants,drug daily dosages(DDDs),the defined daily drug cost(DDC),the drug utilization index(DUI)of 151 hospitals in Guangdong area during 2015 to 2019.Results The consumption sum and DDDs of antidepressants of 151 hospitals in Guangdong increased year by year from 2015 to 2019.The consumption sum and DDDs of selective serotonin reuptake inhibitors(SSRIs),serotonin and norepinephrine reuptake inhibitors(s SNRIs)accounted for nearly 80%of total sales and DDDs.The top five drugs used for five years were escitalopram,flupentixol and melitracen,sertraline,venlafaxine and paroxetine.Agomelatine was the fastest growing,with an average annual growth rate of 194.75%in sales volume and 207.40%in DDDs.TCAs was seldom used,which came bottom of the consumption sum as well as DDDs.The DUI tended to be 1 in each year.The DDC of new antidepressants was higher but decreased year by year.Conclusion The consumption sum of antidepressants and DDDs of 151 hospitals in Guangdong from 2015 to 2019 showed an increasing trend year by year.In recent years,new antidepressants,represented by SSRIs and s SNRIs,have replaced traditional antidepressants and occupied a dominant position in Guangdong.
