摘要
目的探讨PDCA在门急诊处方质量分析中的应用效果,旨在促进门急诊合理用药。方法采用PDCA循环管理模式,从2019年4月~2020年3月的门急诊处方中,每月抽取处方1500张进行点评,分析相关问题产生的原因,并基于此制定干预措施。循环实践3次,并对实施效果进行评价。结果不合格处方率随时间呈现出逐轮次降低趋势,从实施前的5.6%,降到实施3轮后的1.8%,卡方检验分析结果显示,四组间差异具有统计学意义(χ2=119.135,P<0.05)。处方不合理的原因分析结果表明,“诊断模糊或诊断与遴选药品不适宜”所占比例最高,达32.08%;其次为“急诊超3 d用量,门诊超7 d用量未注明原因”占21.60%;“重复用药”所占比例最低,占0.49%。结论采用PDCA对处方进行点评,把不合理情况反馈给临床并执行改进措施是提高处方质量的有效方法。
Objective To explore the application effects of PDCA cycle in prescription quality optimization of outpatient and emergency prescriptions in order to promote the rational use of drugs.Methods 1,500 prescriptions were selected as data from outpatient and emergency prescriptions from April 2019 to March 2020.PDCA cycle management mode was used in the analysis and comment.The reason for related problems were analyzed,and preventive measures were formulated based on this.The implementation effects was evaluated after three PDCA cycles.Results The unqualified prescription rate decreased from 5.6%in the first quarter to 1.8%in the fourth quarter.Chi square test showed that the difference among the four groups was statistically significant(χ2=119.135,P<0.05).The results showed that the proportion of"fuzzy diagnosis or unsuitable diagnosis and drug selection"was the highest,accounting for 32.08%;the second was"three-day overdose of emergency,seven-day overdose of outpatient without reasons",accounting for 21.60%;the lowest proportion was"Repetitive medication",only accounting for 0.49%.Conclusion PDCA used to the prescription comment,feed back the unreasonable situation to the clinic and formulation of corresponding improvement measures,is an effective method to improve the prescription quality.
