动态血压与动态心电图同步监测在高血压患者无症状性心肌缺血中的临床意义 被引量:8

The clinical significance of simultaneous monitoring of ambulatory blood pressure and ambulatory electrocardiography in silent myocardial ischemia of patients with hypertension

摘要 目的:探讨动态血压与动态心电图同步监测在高血压患者无症状性心肌缺血中的应用。方法:选取2018年10月至2019年10月就诊于石河子大学医学院第一附属医院的高血压无症状性心肌缺血患者40例作为观察组,同时选择单纯性高血压患者40例作为对照组,性别不限,年龄37~65岁,均给予动态血压与动态心电图同步检测,观察两组患者心率、动态血压水平与动态心电图监测结果。结果:观察组白昼和夜间心率水平高于对照组[(78±5)次/min比(75±6)次/min,(69±6)次/min比(65±5)次/min,P均<0.05];观察组白昼和夜间收缩压、舒张压水平高于对照组(P<0.01);观察组白昼、夜间的ST段下移水平、持续时间及心肌缺血阈变异值均高于对照组(P<0.01)。结论:动态血压和心电图对高血压患者进行同步监测能够观察血压、心率与心肌缺血之间的关系,为临床提供可靠的诊治依据。 Objective:To explore the application value of simultaneous monitoring of ambulatory blood pressure and ambulatory electrocardiography in silent myocardial ischemia in patients with hypertension.Methods:Forty patients with hypertension and silent myocardial ischemia in First Affiliated Hospital,School of Medicine,Shihezi University,from October 2018 to October 2019,were enrolled in the study,as observation group,40 patients with simple hypertension as control group,both of sexes,37-65 years old.All patients were monitored with ambulatory blood pressure and ambulatory electrocardiography simultaneously.Heart rate,ambulatory blood pressure and ambulatory electrocardiogram were observed.Results:The heart rate of daytime and nighttime in the observation group was higher than that in the control group[(78±5)beats/min vs.(75±6)beats/min,(69±6)beats/min vs.(65±5)beats/min,P<0.05].The levels of systolic and diastolic blood pressure of daytime and nighttime were higher in the observation group than those in the control group(P<0.01).The level and duration of ST segment depression and myocardial ischemia threshold variation were higher in the observation group than those in the control group(P<0.01).Conclusion:The synchronous monitoring of ambulatory blood pressure and electrocardiography in patients with hypertension can observe the relationship among blood pressure,heart rate and myocardial ischemia,and provide reliable basis for clinical diagnosis and treatment.
