摘要
目的:探讨基于加速康复外科(ERAS)理念下的围术期营养支持对老年髋关节置换患者的干预效果。方法:选取2018年1月-2019年10月于本院就诊的老年髋关节置换患者72例,按照随机数字表法分为对照组和研究组,各36例。对照组给予常规围手术期营养支持,研究组给予基于ERAS理念下的围手术期营养支持。比较两组恢复情况、营养水平、髋关节功能及术后并发症发生情况。结果:研究组切口干燥时间、开始主动功能锻炼时间、首次下床时间及住院天数均短于对照组(P<0.05)。出院时与出院后3个月,研究组血清前白蛋白(PA)、白蛋白(Alb)、血红蛋白(Hb)及Harris髋关节功能评分均高于对照组(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:基于ERAS理念下围手术期营养支持能显著促进老年髋关节置换患者恢复,改善营养状况,提高髋关节功能,可临床推广。
Objective:To explore the intervention effect of perioperative nutritional support based on the concept of enhanced recovery after surgery(ERAS)on senile patients with hip arthroplasty.Method:A total of 72 senile patients with hip arthroplasty admitted to our hospital from January 2018 to October 2019 were selected.They were divided into control group and study group according to the random number table method,36 patients in each group.The control group was given conventional perioperative nutritional support,while the study group was given perioperative nutritional support based on the concept of ERAS.The recovery,nutritional level,hip function and postoperative complications were compared between the two groups.Result:The incision drying time,active functional exercise time,first time out of bed and length of stay in the study group were lower than those in the control group(P<0.05).At discharge and 3 months after discharge,the serum prealbumin(PA),albumin(Alb),hemoglobin(Hb)and Harris hip joint function scores in the study group were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Perioperative nutritional support based on the concept of ERAS can significantly promote the recovery of senile patients with hip arthroplasty,improve nutritional status and hip function,which can be clinically promoted.
