氟暴露与成人血清肌酐、尿素氮和尿酸水平的关系 被引量:5

摘要 目的:探讨氟暴露与成人血清肌酐、尿素氮和尿酸水平的关系。方法:随机选取河南省通许县6个村庄为调查点,整群抽取当地居住年限大于5年的18~60岁人群为调查对象。利用氟离子选择电极法和苦味酸法分别测定尿氟和尿肌酐含量,并以尿肌酐作为协变量调整尿氟。随后根据尿氟浓度将调查对象分为高氟暴露组(n=278)和对照组(n=849)。采用罗氏全自动生化分析仪检测调查对象血清中肌酐、尿素氮和尿酸水平。将调查对象按尿氟浓度的四分位间距分成4组,以评估血清肌酐、尿素氮和尿酸随尿氟增加的变化趋势。结果:高氟暴露组人群血清尿素氮[(5.65±1.44)vs(5.19±1.33)mmol/L]及尿酸水平[(281.82±76.78)vs(266.08±79.16)μmol/L]均高于对照组人群(P均<0.05)。多重线性回归结果显示,无论性别分层与否,人群血清尿素氮水平均与尿氟浓度呈正相关(P均<0.05);而血清尿酸水平仅与女性尿氟浓度呈正相关(P<0.05)。结论:氟暴露可引起成人血清尿素氮、尿酸水平改变,且一定程度上存在性别差异。 Aim:To investigate the relationships between fluoride exposure and serum creatinine,urea nitrogen and uric acid in adults.Methods:Six villages in Tongxu County of Henan Province were randomly selected as survey areas.Subjects aged 18-60 years and living in the local area for more than 5 years were included.The concentrations of urinary fluoride and urinary creatinine were determined by fluoride ion selective electrode method and picric acid method,respectively.Then the subjects were divided into high fluoride exposure group(n=278)and control group(n=849)according to urinary fluoride concentration.Serum creatinine,urea nitrogen and uric acid levels were measured using a Roche automatic biochemical analyzer.The subjects were divided into four groups according to the interquartile range of urinary fluoride concentration to evaluate the linear trend of serum creatinine,urea nitrogen and uric acid with increasing urinary fluoride.Results:The levels of serum urea nitrogen[(5.65±1.44)vs(5.19±1.33)mmol/L)]and uric acid[(281.82±76.78)vs(266.08±79.16)μmol/L)]in the high fluoride exposure group were higher than those in control group(P<0.05).Multiple linear regression results showed that serum urea nitrogen levels were positively correlated with urinary fluoride level whether gender stratification was performed or not(P<0.05).However,the positive correlation between serum uric acid level and urinary fluoride concentration was observed only in females(P<0.05).Conclusions:Fluoride exposure can cause changes in serum urea nitrogen and uric acid levels in adults,and there are gender differences to some extent.
